不同剂量A型肉毒毒素治疗小儿脑性瘫痪疗效对照研究

Treatment of Spastic Cerebral Palsy by Local Injection with Botulinum-A Toxin

摘要: 为研究不同剂量A型肉毒毒素(B-TXA)治疗小儿脑性瘫痪痉挛性下肢瘫的效果,将痉挛型下肢瘫患儿120例均分为3组,均采用国产B-TXA进行痉挛肌肉局部注射,用量:1组1U/kg;2组2U/kg;3组3U/kg.应用PRS法,评价治疗前、治疗后2周各组间的疗效差异;随访1、2、3、6个月的疗效。结果:治疗后较治疗前各项PRS指标明显改善,较大剂量组比较小剂量组疗效明显(P<0.01).治疗中及治疗后无1例发生不良反应。结果提示:治疗小儿脑性瘫痪,适当增加B-TXA剂量(≤6U/kg)比较安全,且可取得更好疗效。

Abstract: To evaluate the efficacy of local intramuscular injections of three different doses of botulinum Atoxin (B-TXA) in the management of the spasm of cerebral palsy. Method: 120 cases of children with cerebral palsy were divided into three average groups: group 1, group 2 and group 3. Three different doses of BTX-A (group 1, 1 U/kg; group 2, 2 U/kg; group 3, 3 U/kg) were administered. The spastic muscles were identified by palpitation. The effect of treatment was evaluated by PRS method. The different efficacy between each group was calculated by analysis of variance. The tolerance to BTX-A and the side effects were also assessed. Result: Reduction on spasm of muscles in all patients (100%) became apparent after injection (P<0.01). There is significant differences when the three different doses group was compared one by one (see table). Larger quantity of BTX-A(2 U/kg, 3 U/kg) is more effective than that of less quantities. No major side effects occurred. Larger quantity of BTX Atreating cerebral palsy is more effective than that of smaller quantity. It is safe when the dosages are in a suitable range (≤6 U/kg).
