腰椎间盘手术对腰椎节段矢状面稳定性影响的生物力学研究

摘要: 通过尸体腰椎的生物力学试验,分析了椎间盘手术对腰椎矢状面稳定性的影响,为腰椎间盘手术的改进提供依据。在生理应力范围内,4个腰椎节段的结果显示:①与正常椎间盘产生的位移相比,单纯减压腰椎节段的水平位移略有增加,但无显着性差异。②髓核摘除和刮除软骨板在递增的各负荷下都可以使水平位移显着增加,两者与正常椎间盘产生的位移相比,结果存在极显着性差异(P<0.01).结果提示:①单节段的腰椎间盘髓核摘除术有造成术后在矢状面出现腰椎节段不稳的倾向,在临床上应该给予重视。②椎体间在矢状面上的不稳程度在一定的生理负荷范围内与椎间盘内容物摘除量无关。但随着负荷的增加,过多摘除椎间盘组织,会影响腰椎在矢状面的稳定性.

Abstract: In order to improve the outcome of lumber discectomy, a biomechanical study was performed to identify the effects of posterior approach of lumber operation on the sagittal stability of lumber spine. Comparing with the displacements of normal disc group, the horizontal displacements of disc were slightly increased in the group that underwent decompression on the posterior outer annulus fibrosus(P>0.05), and significant increased in the group that underwent entire nucleus pulposus removal and the group that underwent curettage of cartilage end plate(P<0.01). The results suggested that single level discectomy of lumber spine trends to occur sagittal segmental instability after surgery. This should be given more consideration on the clinical setting.There is no relationship between the extent of sagittal segmental instability and the content of disc removal under low physiological shearing force. When the force gets higher, the outcome goes reversal.
