一组家庭聚集性暴发重症SARS病例及其康复期研究

A Breakout Case of Family Aggregation in Severe Acute Respiratory Syndrome

摘要: 为了解严重急性呼吸综合征(SARS)的家庭聚集性暴发过程,分析SARS不同阶段的传染性和不同接触方式的危险性及预后影响因素,并进行康复期评价。以首发病例为线索,对密切接触者进行个案调查,分析其传播关系和传染性,并对康复者进行肺功能及血清抗体检查。结果:与处于症状期的首发病例密切接触的共8人,均先后发病,死亡1例。发病者潜伏期长短与接触方式及个人感染的病毒载量有密切关系。SARS患者年龄大于60岁、伴基础疾病者病死率高。康复4周时患者仍有不同程度的肺功能障碍,血清IgG达(1.11±0.48)g/L。提示:SARS患者症状期传染性强,未观察到SARS潜伏期具有传染性;年龄大于60岁、伴基础疾病的SARS患者预后较差。

Abstract: The aim was to investigate the family aggregation in the transmission process of severe acute respiratory syndrome(SARS) and to evaluate the infectiveness of SARS patients in different periods of disease epidemics, prognoses related factors and functions in restoration period. The index case is the clue of the investigation, transmission process, infectivity, transmission chain and contact history of SARS were studied through data analysis. Lung function and serum antibodies of recovered patient of SARS was studied. Eight people had a close contact with the index case, all of them were infected, and one of them died. It indicated that SARS case were infectious during their symptom period. Mortality of patients over sixty and/or complicated with other diseases was high. With lung function impediment in some degree, the IgG level was (1.11±0.48)g/L in SARS patients four weeks after recovery. In the investigation, people who had close contact with the index case in symptom period were infected. There is no evidence indicating that SARS case are no infections during their incubation period. The IgG level of recovered SARS patients is high.
