多色流式细胞术对60例成人急性髓系白血病免疫分型的研究

Research on Immunophenotyping with Muti-color Flow Cytometry in Sixty Adults with Acute Myeloid Leukemia

摘要:

目的 探讨成人急性髓系白血病(AML)多色流式细胞术免疫分型的特点。方法 采用CD45/SSC双参数散点图设门方法进行多色流式细胞术免疫表型分析。结果 髓系抗原CD13和CD33在AML各亚型中均有很高的表达,阳性率分别为98.3%和85.0%.CD14和CD64在AML-M4及AML-Ms患者中的表达率较高.CD14和CD64在AML-M4的表达率分别为60.0%和100.0%,在AML-Ms的表达率分别为80.0%和100.0%,但CD14的特异性要高于CD64.AML患者CD34、CD117、HLA-DR的表达率分别为59.3%、84.3%和71.7%;AML-M3患者CD117表达率为76.9%,CD34及HLA-DR呈低表达.AML患者淋系抗原CD7和CD19阳性率分别为20.0%和3.3%。结论 采用CD45/SSC双参数散点图设门方法,使成人AML免疫分型结果更可靠.CD13、CD33、CD14可作为AML免疫分型的髓系单抗.CD34及HLA-DR低表达为AML-M3的特征.成人AML淋系抗原表达以CD7较常见.

Abstract:

Objective To evaluate the charateristics of immunophenotyping in adults with acute myeloid leukemia(AML) using multi-color flow cytometry.Methods Immunophenotyping was performed with multi-color flow cytometry using CD45 /SSC gating.Results The myeloid antigen of CD13 and CD33 was highly expressed in all patients with AML. The positive rate was 98.3 % and 85.0% respectively. The expression of CD14 and CD64 in patients with AML-M4 and AML-M5 were also high. The positive rate was 60.0%, 100.0% in AML-M4 and 80.0%, 100.0% in AML-M5 respectively. The expression of CD34 、CD117 and HLA-DR in patients with AML was 59.3%, 84.3% and 71.7% respectively. CD117 was expressed in 76.9% of patients with AML-M3. Expression of CD34 and HLA-DR was low in patients with AML-M3. CD7 and CD19 expression in patients with AML was 20.0% and 3.3% respectively.Conclusion The immunophenotyping results of adults with AML were more reliable with gating of CD45 /SSC. Monoclonal antibodies of CD13, CD33 and CD14 can be used to detect the immunophenotyping in AML. The expression of CD34 and HLA-DR was low in patients with AML-M3. The expression of CD7 lymphoid-associated antigens is relatively common in adults with AML.
