血清残粒脂蛋白胆固醇浓度与急性心肌梗死相关性分析

Correlation between residual lipoprotein cholesterol and acute myocardial infarction

摘要: 目的  通过对比分析血脂浓度在冠状动脉粥样硬化性心脏病(以下简称冠心病)不同亚组患者中的浓度,探讨残粒脂蛋白胆固醇(residual lipoprotein-cholesterol,RLP-C)与急性心肌梗死(acute myocardial infarction,AMI)的相关性。方法  选取2020年1月至2021年1月首都医科大学附属北京朝阳医院心内科收治的冠心病患者为研究对象,将其中266例AMI患者作为病例组(AMI组),267例非AMI患者为对照组(非AMI组),开展病例对照研究。基于公式计算出RLP-C指标和血浆致动脉粥样硬化指数(atherogenic index of plasma, AIP)。通过单因素分析及多因素Logistic回归模型,分析RLP-C浓度与AMI的关系。结果  单因素分析结果显示, AMI组患者RLP-C浓度显著高于非AMI组患者,组间差异有统计学意义(P<0.05)。多因素Logistic回归分析显示,在控制了其他因素的干扰作用后,RLP-C仍然为AMI的独立危险因素(P <0.05),RLP-C浓度升高的患者,AMI的危险性增大,并且在诸多影响因素中,RLP-C与AMI关系最为密切(OR =3.093,95%CI:1.658~5.771)。结论  血清RLP-C浓度与急性心肌梗死有关,RLP-C浓度升高的患者发生AMI的风险增大。

Abstract: Objective  To investigate the correlation between residual lipoprotein-cholesterol (RLP-C) and acute myocardial infarction (AMI) by analyzing the changes of blood lipid levels in different subgroups of patients with coronary heart disease.Methods  Patients with coronary heart disease who were admitted to the Department of Cardiology, Beijing Chaoyang Hospital, Capital Medical University from January 2020 to January 2021 were selected as the study objects. The total of 266 patients with AMI were selected as the case group (AMI group) and 267 patients with non-AMI were selected as the control group (non-AMI). The RLP-C index and  atherogenic index of plasma(AIP) were calculated based on the formula. The relationship between RLP-C level and AMI was analyzed by univariate analysis and multiple Logistic regression model.Results  The results of univariate analysis showed that the level of RLP-C in AMI group was significantly higher than that in non-AMI group, and the difference was statistically significant (P<0.05). Multivariate Logistic regression showed that after controlling the interference effect of other factors, RLP-C was still an independent risk factor for AMI (P<0.05). Patients with elevated levels of RLP-C had an increased risk of AMI, and among many influencing factors, RLP-C was most closely related to AMI (OR=3.093, 95% CI: 1.658-5.771).Conclusions  Serum RLP-C level is associated with AMI, and the risk of AMI increases in patients with elevated RLP-C level.
