综合心理干预对稳定期精神分裂症患者执行功能及社会功能的影响

摘要： 目的探讨综合心理干预对改善稳定期精神分裂症患者执行功能和社会功能的效果。方法纳入80例稳定期精神分裂症患者随机分为2组。试验组接受综合心理干预，对照组只接受医院提供的一般药物治疗以及常规心理相关知识宣教。在基线、干预结束后（第12周）评估症状严重程度、执行功能和社会功能。结果治疗12周后，试验组的阳性和阴性症状量表（Positive and Negative Syndrome Scale，PANSS）总分、阴性症状量表和一般精神病理量表分均低于对照组（P<0.05）；威斯康星卡片分类测试（Wisconsin Card Sorting Test，WCST）持续性错误数和非持续性错误数低于对照组（P<0.05），连线测验(Trail Making Test，TMT)中A、B测验反应时间低于对照组（P<0.05），Stroop色词测验（Stroop Color Word Test, CWT）卡片A、B、C耗时、反映干扰量（stroop interference effect，SIE）耗时数和正确率低于对照组（P<0.05），卡片A、B、C正确率高于对照组（P<0.05）；个体和社会功能量表（Personal and Social Performance Scale，PSP）评分高于对照组（P<0.05）。结论综合心理干预有助于减少稳定期精神分裂症患者的阴性症状，改善执行功能，提高社会功能。

Abstract: Objective To explore the effect of comprehensive psychological intervention on improving executive and social functioning in patients with stable schizophrenia. Methods Eighty patients with stable schizophrenia were included and randomly divided into 2 groups. The experimental group received comprehensive psychological intervention. The control group received only general medication provided by the hospital and routine psychological-related knowledge education. Symptom severity, executive functioning, and social functioning were assessed at baseline, and at the end of the intervention (week 12). Results After 12 weeks of intervention, the total score, negative scale, and general psychopathology scale scores of Positive and Negative Syndrome Scale(PANSS) in the experimental group were lower than those in the control group（P<0.05）. The numbers of Rpe and nRpe in Wisconsin Card Sorting Test （WCST） were lower than that in the control group（P<0.05）. The reaction times of TMT-A and B were lower than that of the control group（P<0.05）. The time consumption, stroop interference effect(SIE) time consumption, and SIE accuracy of cards A, B, and C in Stroop Color Word Test (CWT） were lower than those in the control group（P<0.05）, while the accuracy of cards A, B, and C were higher than that in the control group（P<0.05）. The score of Personal and Social Performance Scale(PSP) was higher than that of the control group（P<0.05）. Conclusions Comprehensive psychological intervention can help reduce negative symptoms, improve executive functioning, and enhance social functioning in patients with stable schizophrenia.
