纯音测听在糖调节异常患者听功能检测中的应用

摘要: 目的 通过检测糖调节异常(impaired glucose regulation, IGR)患者的纯音测听情况,探讨其在IGR患者听觉功能筛查中的价值.方法 口服葡萄糖耐量试验组筛查入选无耳聋症状的IGR患者30例,同时选取年龄性别匹配正常人30例作为对照组.对入选人群行纯音测听、声导抗检查.结果 ①IGR组30例中11例(36.67%)存在听力损失,正常对照组30例中3例(10%)存在听力损失,两者比较差异有统计学意义(χ2=5.963,P=0.015).②IGR组各频率纯音听阈均较对照组升高,在4 000 Hz及8 000 Hz差异有统计学意义(P=0.008,P=0.003).③IGR组有耳鸣症状者11例,其中伴听力损失3例,与无耳鸣症状者比较,两组听力损失发生率差异无统计学意义(P=0.466).结论 IGR患者听力损失发生率高,且表现为高频听力受损,听力损失可发生于耳聋症状前.

Abstract: Objective To explore the value of the pure-tone audiometry in auditory function screening among the impaired glucose regulation(IGR) patients. Methods ①Thirty IGR patients without the symptoms of deafness and 30 controls were enrolled in the study. Pure tone audiometry and tympanometry were tested (evaluated). Results The prevalence rate of hearing loss was higher in the IGR group (33.67%) than that in the control group (10%, P<0.05). ②As compared with the control group, the hearing threshold at each frequency in the IGR group was higher, but there were statistically significant differences only at 4 000 and 8 000 Hz (P=0.008, P=0.003). ③Eleven cases in the IGR group had the symptoms of tinnitus, and among them 3 cases had hearing loss. There was no significant difference in the prevalence rate of hearing loss between the cases with hearing loss and those without hearing loss in IGR group (P=0.466). Conclusion The prevalence rate of hearing loss in patients with IGR was significantly increased, and high-frequency hearing loss was more profound. Hearing loss in IGR may occur before the symptom of deafness.
