摘要: 目的  探讨医源性早发性卵巢功能不全(premature ovarian insufficiency,POI)对脂代谢的影响。方法  本研究募集了2021年5月到2022年11月就诊于首都医科大学附属北京妇产医院内分泌科的94例医源性POI患者为观察组,及同时期进行妊娠前准备的健康育龄期女性94名作为对照组。测量两组患者的身高、体质量,空腹抽血测定总胆固醇(total cholesterol,TC)、三酰甘油(triglyceride,TG)、高密度脂蛋白胆固醇(high density lipoprotein-cholesterol,HDL-C)、低密度脂蛋白胆固醇(low density lipoprotein-cholesterol,LDL-C)、卵泡刺激素(follicle stimulating hormone,FSH)、黄体生成素(luteinizing hormone,LH)和雌二醇(estradiol,E2)。结果  观察组女性的体质量指数(body mass index, BMI)、年龄和E2浓度显著低于对照组(P均<0.001)。观察组的FSH、LH、TC、TG和LDL-C浓度显著高于对照组(P均<0.05)。两组间HDL-C浓度差异无统计学意义(P =0.924)。在校正BMI和年龄后,结果没有变化。观察组患者血脂异常的发生率显著高于对照组(P=0.022)。结论  医源性POI患者血脂异常的比例明显高于同龄育龄期女性,提示医务工作者应重视医源性POI患者的慢性疾病管理工作,给予适当的个体化治疗措施以降低患者未来心血管疾病的发生风险,尤其要注重在放射治疗和化学药物治疗之前采取以卵巢组织冻存(ovarian tissue cryopreservation,OTC)为核心的生育力保护措施。

Abstract: Objective  To explore the effect of iatrogenic premature ovarian insufficiency on lipid metabolism. Methods  In this study, 94 patients with iatrogenic premature ovarian insufficiency(POI) who visited the Department of Gynecological Endocrinology, Beijing Obstetrics and Gynecology Hospital, Capital Medical University from May 2021 to November 2022 were enrolled as observation group, and 94 healthy women of childbearing age who underwent preconception preparation during the same period were recruited as control group. The height and weight of both groups were measured, and fasting blood was drawn to determine total cholesterol (TC), triglycerides (TG), high density lipoprotein-cholesterol (HDL-C), low density lipoprotein-cholesterol (LDL-C), follicle stimulating hormone (FSH), luteinizing hormone (LH) and estradiol (E2) were measured. Results  Body mass index (BMI), age and E2 levels were significantly lower in patients with iatrogenic POI (all P < 0.001). TC, TG, LDL-C, FSH and LH levels were significantly higher in observation group (all P < 0.05). There was no significant difference in HDL-C level between two groups(P =0.924).  After correction for BMI and age,the results did not change. The incidence of dyslipidemia was significantly higher in observation group. Conclusions  The proportion of dyslipidemia in patients with iatrogenic POI was significantly higher than that in women of the same reproductive age, suggesting that medical practitioners should pay attention to the chronic disease management of patients with iatrogenic POI and give appropriate individualized treatment measures to reduce the risk of future cardiovascular disease in these patients. More importantly, fertility preservation measures centered on ovarian tissue cryopreservation (OTC) should be taken prior to radiotherapy and chemotherapy.
