神经外科亚低温麻醉中局部脑皮质温度的监测

Direct Intraoperative Measurement of Local Brain Temperature in Neurosurgical Patients during Mild Hypothermia

摘要/Abstract

摘要： 监测神经外科亚低温麻醉中局部脑皮质温度,并与食管及鼻咽部温度比较,以探讨影响局部脑温度的因素.对10例择期神经外科手术患者(ASA1～2级),术中采用降温毯将食管温度降至33～35℃,术中持续监测食管、鼻咽部、术野局部脑皮质温度.术中剪开硬膜后,将21G针状脑皮质温度探头插入脑皮质1cm,直至硬膜缝合前取出.同时记录出入量、室温.结果:在同一时间点,鼻咽温度明显低于食管温度,最大温度差达0.37℃;脑皮质局部温度明显低于食管温度,最大温度差可达2.7℃;局部脑皮质温度与鼻咽及食管温度的相关系数分别为0.927、0.935(P<0.01).经多元回归分析,食管温度、体表面积及硬膜开放时间是影响局部脑皮质温度的主要因素;术后24h随访病人,未见与测量温度相关并发症发生.结果提示:神经外科亚低温麻醉中食管及鼻咽部温度不能准确反映局部脑皮质温度,但三者间具有良好的相关性.

Abstract: The objective was to study the relationship of local brain temperature and temperatures measured at standard extracranial monitoring sites in neurosurgical patients during mild hypothermia. In ten ASA patients(gradeⅠ～Ⅱ)scheduled for craniotomy, the esophageal temperature(t_eso), nasopharyngeal temperature(t_nas)and local brain surface temperature(t_brain)were monitored continuously. Local t_brain was monitored at dura opening. At dura opening, local t_brain was lower than t_eso and t_nas(P<0.01), The maximal difference was 2.7 ℃. Local t_brain correlated closely to esophageal(r=0.935)and nasopharyngeal temperature(r=0.927). Local t_brain was mainly influences by esophageal temperature, body surface and time, since the brain surface was exposed was to air. Local t_brain correlated closely to esophageal and nasopharyngeal temperature at dura opening in neurosurgical patients during mild hypothermia, but it is necessary to measure local t_brain for some critical neurosurgery directly.
