氢氯噻嗪、氯沙坦单用或联用降压效果的观察及其对内皮功能的影响

Effects of Hydrochlorothiazide, Losartan and Hyzaar on Decreasing Blood Pressure and Improving Endothelial Function

摘要: 为比较氢氯噻嗪、氯沙坦、氯沙坦/氢氯噻嗪(海捷亚)3种药物对轻中度原发性高血压患者诊室血压和内皮功能的影响,选择77例初诊原发性高血压患者随机服用氢氯噻嗪、氯沙坦、氯沙坦/氢氯噻嗪,分别在治疗前和治疗4周后测量诊室血压及血浆内皮素(ET)和一氧化氮(NO)浓度.发现3种药物均有显著降低诊室血压的作用,氯沙坦/氢氯噻嗪降舒张压幅度和降平均动脉压幅度优于其他2组.氯沙坦/氢氯噻嗪明显升高血浆NO浓度和NO/ET比值.提示:氯沙坦/氢氯噻嗪是一种良好的复合降压制剂,降低血压的效果优于氯沙坦和氢氯噻嗪;氯沙坦/氢氯噻嗪能改善轻中度高血压患者的内皮功能.

Abstract: To compare the effects of hydrochlorothiazide(HCTZ) 12.5 mg, losartan 50 mg and Hyzaar (losartan 50 mg/HCTZ 12.5 mg) on decreasing blood pressure and improving endothelia function in patients with essential hypertension, 77 patients with mild or moderate essential hypertension were arranged into three randomized, parallel groups for 4 weeks' treatment with HCTZ, losartan or losartan/HCTZ respectively. Before and after the treatment, seating blood pressure and endothelia function were measured. The result showed that seating blood pressure in groups of HCTZ, losartan or Losartan/HCTZ decreased significantly after antihypertensive treatment compared with before. The reducing degree of the diastolic blood pressure and the average artery blood pressure of group HCTZ and Losartan were significantly lower than that of group losartan/HCTZ. It is suggested that losartan/HCTZ is a good compound antihypertensive drug, which has better effects on decreasing blood pressure and improving endothelial function than that of the HCTZ or Losartan in patients with essential hypertension.
