快速动眼睡眠行为障碍疾病与阻塞性睡眠呼吸暂停综合征关系的多导睡眠监测特征研究

Polysomnographic characteristics of obstructive sleep apnea syndrome complicated with rapid eye movement (REM) sleep behavior disorder

摘要:

目的 研究快速动眼睡眠行为障碍疾病(rapid eye movement sleep behavior disorder,RBD)与阻塞性睡眠呼吸暂停综合征(obstructive sleep apnea syndrome,OSAS)的关系。方法 选取2012年1月1日至2013年10月31日就诊于首都医科大学宣武医院睡眠门诊且呼吸暂停低通气指数(apnea-hypopnea index,AHI)>10/h的OSAS患者76人,其中OSAS合并RBD组28人,单纯OSAS组48人,所有受试者均行一整夜视频多导睡眠仪监测(video-polysomnography,vPSG)检查并对其呼吸参数进行比较分析。结果 RBD-OSAS组AHI、非快速动眼睡眠(non rapid eye movement, NREM)期 AHI、快速动眼睡眠(rapid eye movement, REM)期AHI及低通气指数低于OSAS对照组,基础血氧饱和度及最低血氧饱和度则高于OSAS对照组,低通气最长时间及血氧饱和度低于90%的时间比OSAS对照组短。结论 RBD可能通过提高NREM期及REM期上气道肌肉紧张性活动对同时合并的OSAS发挥保护作用,从而为治疗OSAS提供新思路。

Abstract:

Objective To explore the relationship between REM sleep behavior disorder(RBD) and obstructive sleep apnea syndrome(OSAS).Methods Totally 76 OSAS subjects defined by AHI apnea-hypopnea index>10/h were enrolled from sleep disease center of Xuanwu Hospital from 2012 to 2013(28 RBD-OSAS cases and 48 OSAS controls). All subjects were undergone by one night video-polysomnography(vPSG) and the sleep apnea related parameters were compared between the two groups. Results Compared to OSAS group, RBD-OSAS patients had lower AHI index and hypopnea index, more severity of oxygen desaturation(baseline SpO2 and minimum SpO2) and longer maximum hypopnea time and <90% O2 saturation duration(P<0.05).Conclusion RBD is capable of protecting against OSAS by increasing EMG activity of upper airway muscle tone during both REM and NREM sleep in RBD with OSAS patients. This result may provide a new direction for the treatment of OSAS.
