体质量指数与下肢继发性淋巴水肿预后的分析

Analysis of body mass index and the prognosis of secondary lymphedema in lower extremities

摘要: 目的 研究体质量指数(body mass index,BMI)对下肢继发性淋巴水肿肿胀程度和手术治疗预后的影响。方法 分析2014年10月至2016年3月间于首都医科大学附属北京世纪坛医院行淋巴水肿手术治疗的96例下肢继发性淋巴水肿患者的临床资料,根据BMI分为正常组(BMI 18.5~23.9 kg/m2)25例、超重组(BMI 24.0~27.9 kg/m2)38例、肥胖组(BMI≥28.0 kg/m2)33例,比较3组患者术前及术后6个月患肢和健肢的周径差及体积差等临床资料。结果 3组患者的一般情况差异均无统计学意义(P>0.05);而3组患者术前与术后6个月的患肢和健肢周径差、体积差差异均有统计学意义(P<0.001),肿胀均较术前明显缓解;肥胖组和正常组的术前患肢和健肢体积差、周径差、吸出脂肪量差异均有统计学意义(P<0.05);超重组、肥胖组与正常组术后6个月的患肢和健肢体积差和周径差差异均有统计学意义(P<0.05)。结论 BMI与下肢继发性淋巴水肿肿胀程度和预后有密切关系,是影响肿胀程度和手术预后的重要指标之一。

Abstract: Objective To study the effect of body mass index (BMI) on the degree of swelling and prognosis of patients with secondary lymphedema of lower extremities. Methods Retrospective analysis of the clinical data of 96 patients with secondary lymphedema of lower extremities at Department of Lymph Surgery, Beijing Shijitan Hospital, Capital Medical University from October 2014 to March 2016.They were divided into normal group (BMI 18.5-23.9 kg/m2,25 cases), overweight group (BMI 24.0-27.9 kg/m2,38 cases) and obesity group (BMI ≥ 28.0 kg/m2,33 cases). The clinical data of preoperative and postoperative were compared among the three groups. Results There were no significant differences in the general data of the three groups (P>0.05). Circumference and volume difference between pre-operation and 6 months after operation were significantly different (P<0.001) in three groups. There was a significant difference in fat aspiration, preoperative volume difference, preoperative circumference difference, volume difference of 6 months after surgery and circumference difference of 6 months after surgery between the obesity group and the normal group. There was a significant difference in volume difference of 6 months after surgery,and circumference difference of 6 months after surgery between the overweight group and the normal group. Conclusion BMI is associated with the degree of swelling and prognosis of secondary lymphedema in the lower extremities, and is an important predictor of the severity and surgical outcomes.
