中老年男性2型糖尿病患者肌少症与骨密度的相关性研究

摘要： 目的探讨中老年男性2型糖尿病（type 2 diabetes mellitus，T2DM）患者肌少症与骨密度的相关性。方法纳入首都医科大学宣武医院内分泌科218例≥55岁的男性2型糖尿病住院患者，测定骨密度（bone mineral density，BMD）、四肢骨骼肌肌肉质量（appendicular skeletal muscle mass，ASM）、四肢脂肪含量（appendicular fat mass，AFM）、握力、步速等，并计算出四肢骨骼肌质量指数（appendicular skeletal muscle mass index，ASMI）。分析肌少症与骨质疏松症的相关性。结果 55岁以上男性T2DM患者肌少症患病率为34.86%，低骨骼肌含量患者BMD明显低于非低骨骼肌含量者。多元Logistic回归分析显示，肌少症为中老年男性2型糖尿病患者骨质疏松的独立危险因素（OR=2.76，95%CI： 1.20~6.34，P＝0.017）。结论中老年男性T2DM患者的肌少症与骨质疏松症密切相关。

Abstract: Objective To explore the relationship between sarcopenia and bone mineral density (BMD) in middle-aged and aged men with type 2 diabetes mellitus (T2DM). Methods All 218 men with T2DM aged over 55 years were selected from the Department of Endocrinology of Xuanwu Hospital. The BMD of lumbar spine and hip, appendicular skeletal muscle mass (ASM), appendicular fat mass (AFM), appendicular skeletal muscle mass index (ASMI), body mass index(BMI) were measured. The association between sarcopenia and osteoporosis was assessed. Results The prevalence of sarcopenia in men aged over 55 years with T2DM was 34.86%. The BMD of the participants in sarcopenia group was significantly lower than that in non-sarcopenia group. Multivariate Logistic regression analysis showed sarcopenia was an independent risk for osteoporosis in men with T2DM(OR=2.76, 95% CI: 1.20~6.34, P=0.017). Conclusion The sarcopenia is closely related to osteoporosis in middle-aged and aged men with T2DM.
