老年2型糖尿病患者血尿酸浓度与代谢综合征的相关性分析

Correlation of serum uric acid level and metabolic syndrom in elder patients with type 2 diabetes mellitus

摘要: 目的 探讨老年2型糖尿病患者血清尿酸与代谢综合征的关系。方法 回顾分析首都医科大学附属北京同仁医院内分泌科2018年1月至2020年12月住院的612例老年2型糖尿病患者,其中合并代谢综合征(metabolic syndrome,MS)组439例,不合并MS(non-metabolic syndrome,NMS)组173例。比较两组基本临床资料及血清尿酸浓度;比较不同代谢综合征组分的尿酸浓度;使用Person相关法分析血尿酸与MS各组分因子之间的关系;使用Logistic回归模型,进行血尿酸与MS关系的多因素分析;使用受试者工作特征(receiver operating characteristic,ROC)曲线观察血尿酸预测MS的作用,并筛选截断值。结果 MS组血清尿酸浓度显著高于NMS组(P<0.05),随着MS组分数目增加,尿酸浓度升高;Person相关分析结果显示,尿酸与高密度脂蛋白胆固醇(high density lipoprotein-cholesterol, HDL-C)相关(r=-0.233,P=0.000);多因素Logistic 回归分析结果显示,尿酸(uric acid,UA)是MS的独立危险因素(OR=1.005,P<0.05)。ROC 曲线分析得出,男性血尿酸预测MS截断值为336.50,女性血尿酸预测MS的截断值为317.00。结论 老年2型糖尿病患者血清尿酸浓度与代谢综合征相关,老年2型糖尿病患者血清尿酸浓度升高,发生代谢综合征的危险性增大。

Abstract: Objective To investigate the association of serum uric acid (UA) and metabolic syndrom(MS) in elder patients with type 2 diabetes mellitus(T2DM). Methods From January 2018 to December 2020, a total of 612 elder patients with T2DM were enrolled. Among them, there were 439 patients combined with MS group and 173 patients without MS (non-MS group). The general clinical characteristics and serum UA of the two groups were analyzed.Levels of serum UA in different MS group were compared with each other, while the relationship between serum UA and MS was analyzed with Pearson correlation analysis.Risk factors of MS were analyzed by Logistic regression analysis.A model for UA was established with the receiver operating characteristic (ROC) curve to predict MS incidence and to screen the cut-off value. Results The level of serum UA in MS group was significantly higher than that in Non-MS group (P<0.05). The level of UA was increased with the MS component items increase; The Pearson correlation analysis indicated that serum UA was negatively correlated with high density lipoprotein-cholesterol (HDL-C) (r=-0.233, P=0.000). Logistic regression analysis showed that HDL-C was a protective factor for MS. The ROC curve analysis of serum UA for predicting MS in men yielded a cut-off value of 336.50, and in women yielded a cut-off value of 317.00. Conclusions In elder patients with T2DM, the levels of serum uric acid were associated with metabolic syndrome. The level of serum UA and the risk of developing MS was risen in elderly patients with T2DM.
