细胞角蛋白的表达水平与结直肠癌临床病理学特征的相关性研究

Correlation between the expression of cytokeratin in colorectal cancer and clinicopathological features

摘要: 目的 研究细胞角蛋白(cytokeratin,CK)在结直肠癌组织中的表达水平及其与临床病理特征的关系。方法 收集行手术切除并经病理确诊的结直肠癌组织蜡块标本200例,同时选取其对应的癌旁正常组织作为对照。采用免疫组织化学染色法检测结直肠癌组织与癌旁组织中CK7、8、18~20的表达情况,并统计分析其表达水平与结直肠癌临床病理参数之间的关系。 结果 CK7在癌组织和癌旁组织中都缺乏表达,CK8、18~20在癌组织中表达阳性率均高于癌旁组织,差异具有统计学意义(P<0.05)。CK8在癌组织中的表达与肿瘤的浸润深度正相关(P<0.05);CK19的表达与肿瘤淋巴结转移正相关(P<0.05);CK20的表达与肿瘤分化程度负相关(P<0.05)。结论 CK8、19、20的表达水平分别与肿瘤的浸润、淋巴结转移和分化程度密切相关,对临床上判断结直肠癌的恶性程度具有一定的指导意义。

Abstract: Objective To explore the expression of cytokeratin in colorectal cancer and its relationship with clinicopathological features. Methods A total of 200 specimens with colorectal carcinoma confirmed by surgery and postoperative pathology were selected, and corresponding adjacent normal tissues were selected as controls. The expression of CK7,8,18-20 in colorectal cancer tissues and in adjacent normal tissues were detected by immunohistochemistry. Their correlation with pathological characteristics and general information was compared. Results CK7 was not expressed in cancer tissues and adjacent normal tissues. The positive expression rates of CK8 and 18-20 in tumor tissues were significantly higher than those in the corresponding adjacent normal tissue (all P<0.05). The expression of CK8 in cancer tissue was positively correlated with the depth of tumor invasion. The expression of CK19 was positively correlated with lymph node metastasis. The expression of CK20 was negatively correlated with the degree of tumor differentiation. Conclusion The expression levels of CK8, 19 and 20 are closely related to tumor invasion, lymph node metastasis and differentiation. The expression levels of CK8, 19 and 20 are significant in judging the malignant degree of colorectal cancer.
