空腹血糖、OGTT2h血糖和糖化血红蛋白同糖尿病视网膜病变的相关性比较

Comparison for Association of Fasting and 2-hour Glucose and HbA1c with Diabetic Retinopathy

摘要: 目的 比较空腹血糖(fasting plasma glucose,FPG)、口服葡萄糖耐量实验(oral glucose tolerance test,OGTT)2 h血糖(2-h plasma glucose,2hPG)和糖化血红蛋白(hemoglobin A1c,HbA1c)同糖尿病视网膜病变(diabetic retinopathy,DR)的相关性,评价诊断糖尿病的最佳方法。方法 在北京市昌平区进行了一项有关慢性疾病和危险因素的健康调查,其中2 551人进行了OGTT、HbA1c测定和眼底检查。采用受试者工作特征(receiver operating characteristic,ROC)曲线评价FPG、2hPG 和HbA1c同DR的相关性,通过曲线下面积(area under curve,AUC)的比较判断诊断糖尿病的最佳方法。结果 与无DR组相比,合并DR组FPG、2hPG、HbA1c、血压水平以及合并高血压的比例显著升高(P<0.05 或P<0.001)。2hPG的曲线下面积(AUC)为86.9%(95%CI:82.2~91.7),和HbA1c的AUC为86.4%(95%CI:80.8~92.0)相比差异无统计学意义(P=0.796),和FPG的AUC为85.4%(95%CI:80.0~90.7)相比差异无统计学意义(P=0.501)。在排除接受降糖药物治疗的人之后,这3项血糖指标在检测DR方面也无显著差异。结论 在中国人群中用FPG或HbA1c诊断糖尿病,同2hPG同样有效可靠。

Abstract: Objective To compare the association of fasting plasma glucose(FPG), 2-h plasma glucose(2hPG) and hemoglobin A1c(HbA1c) levels with diabetic retinopathy(DR) and to evaluate the best way to diagnose diabetes. Methods A cross-sectional, population-based survey on chronic diseases and risk factors was conducted in Changping; in 2 551 subjects, the three glycemic measures in oral glucose tolerance tests(OGTT), and DR was assessed by digital retinal images. Receiver operating characteristic(ROC) curves were calculated and compared the areas beneath them to compare the ability of FPG, 2hPG and HbA1c measurements to detect DR. Results Patients with DR were rated significantly higher in FPG, 2hPG, HbA1c, blood pressure and subjects suffered from hypertension compared with subjects without DR(P<0.05 or P<0.001). The area under the ROC curve for 2hPG was 86.9%(95% CI: 82.2~91.7) and was not significantly larger than that for FPG 85.4%(95% CI: 80.0~90.7; P=0.501) and that for HbA1c 86.4%(95% CI: 80.8~92.0; P=0.796). After excluding individuals receiving antihyperglycemic medication, all three measures of glycemia also showed no significant differences for detecting DR. Conclusion Measuring FPG or HbA1c are equally reliable methods in measuring 2hPG for the diagnosis of diabetes in the Chinese population.
