京津冀地区健康人呼出气一氧化氮正常值调查研究

Study of fractional exhaled nitric oxide value in healthy subjects of Beijing-Tianjin-Hebei Region

摘要: 目的 调查京津冀地区健康成年人呼出气一氧化氮浓度(fractional exhaled nitric oxide,FeNO)正常值范围。方法 选取在首都医科大学附属北京同仁医院门诊陪诊且符合健康标准的成年人523名(男性221名,女性302名),采用NIOX测定FeNO值,并探讨其与年龄、身高、体质量、体质量指数及地域等相关因素的关系。结果 京津冀地区健康成年人FeNO均值为16×10-6,其95%正常值区间(偏态分布)为(5~26)×10-6,与成人的年龄分层、性别及地区差异有统计学意义(P<0.05),而与身高、体质量等因素差异无统计学意义(P>0.05)。FeNO均值地区分布差异有统计学意义(P<0.05),北京地区正常值(经对数-反对数转换)最低(14.6±6.3),天津地区次之(16.0±5.9),而河北地区最高(17.6±5.6);其95%正常值范围(偏态分布)分别为(5~26)×10-6、(5~26)×10-6、(5~27)×10-6。京津冀地区男女性别的差异导致其均值及95%置信区间均有差异。结论 京津冀地区健康成年人的FeNO值存在地区差异,26×10-6作为京津冀地区健康成年人FeNO的95%正常值范围,应考虑地区、年龄、性别等因素的影响。

Abstract: Objective To establish measuring fractional exhaled nitric oxide (FeNO) provides an indication of airway inflammation and is used as an inflammatory marker for airway inflammation management. However, reference values and determinants of FeNO levels are not well defined in local healthy adults reference values in nonsmoking, healthy adults and to determine the factors related to FeNO levels in Beijing-Tianjin-Hebei regions.Methods This randomized multi-center study was carried on in twenty medical centers; 523 nonsmoking, healthy adults (male 221, female 302) were involved in the same period. FeNO was measured in healthy adults according to American Thoracic Society Guidelines (single breath online, exhalation flow 50 mL/s) with NIOX exhaled nitric oxide monitoring system (NIOX, Aerocrine AB, Sweden), and the correlation between related factors[such as:age, height, weight, body mass index (BMI), region and lung function] and FeNO level were investigated. Lung function was measured using spirometry, and atopic status was excluded based on the skin-prick test.Results FeNO normal values in healthy adults are similar compared with previous reports. The geometric mean of FeNO in 523 healthy adult was 16×10-6, and 95% confidence interval (skewed distribution) was (5-26)×10-6. FeNO correlated significantly with age, gender and regional distribution (P<0.05). However, weight, height, and BMI did not produce a significant impact on adult FeNO value (P>0.05). The normal ranges of FeNO were (5-26)×10-6 in healthy adults of our study.Conclusion The result of this study suggests that 26×10-6 seems to be the reference FeNO value in healthy adults in Beijing-Tianjin-Hebei regions. We believe that the presented FeNO reference values and the determining factors could be useful for research and clinical practice in the adult population of Beijing-Tianjin-Hebei regions. However, influencing factors such as age, gender and region must be considered.
