妊娠中期胎儿心轴角度与胎儿先天性心脏病的关系

Relationship between fetal cardiac axis angle and congenital heart defects in the second trimester of pregnancy

摘要: 目的 通过测量胎儿心脏角度,探讨妊娠中期胎儿心轴角度与胎儿先天性心脏病(congenital heart defects,CHD)之间的关系。方法 选择2017年12月至2019年5月在首都医科大学附属北京妇产医院建档并进行规范化产前超声筛查的胎儿为研究对象,其中病例组52例,对照组104例。按照国际妇产超声学会(International Society of Ultrasound in Obstetrics and Gynecology,ISUOG)指南标准,在胎儿四腔心切面测量胎儿心轴角度。结果 病例组与对照组间孕妇年龄及胎儿胎龄差异无统计学意义(t=1.825、-0.928,P>0.05)。病例组心轴角度为50.03°±24.02°,对照组心轴角度为42.64°±6.30°,差异具有统计学意义(P<0.05)。病例组心轴异常所占比例为30.77%(16/52),对照组心轴异常所占比例为0.96%(1/104),差异具有统计学意义(P<0.05),心轴角度异常的胎儿发生先天性心脏病的风险增加了近45倍,表明胎儿心轴角度异常与胎儿先天性心脏病的关系十分密切。结论 胎儿心轴异常与胎儿先天性心脏病密切相关,当超声筛查发现心轴异常时,应更加注意胎儿心脏的检查。

Abstract: Objective To explore the relationship between fetal heart axis angle and the fetal congenital heart disease (CHD) in the second trimester of pregnancy by measuring the angle of fetal heart axis. Methods There were 52 fetuses in the abnormal group and 104 in the control normal group were selected as the research objects, all the pregnant women were taken maternal and infant health records and taken standardized prenatal ultrasound screening in Beijing Obstetrics and Gynecology Hospital, Capital Medical University from December 2017 to May 2019. According to the International Society of Ultrasound in Obstetrics and Gynecology (ISUOG) practice guidelines for sonographic screening examination of fetal heart, the fetal axis angle was measured in the four-chamber view of the fetus. Results There was no significant difference in maternal age and fetal age between the abnormal group and the control group (t=1.825、-0.928,P>0.05). The cardiac axis angle of abnormal group was (50.03°±24.02°), and that of control group was (42.64°±6.30°) respectively. The difference was statistically significant between the two groups (P<0.05). The proportion of abnormal axis was 30.77% (16/52) in abnormal group and 0.96% (1/104) in control group, with statistical significance (P<0.05). The risk of CHD in fetuses with abnormal axis angle increased by nearly 45 times, it suggested that the n abnormal fetal heart axis angle is closely related to CHD. Conclusion Abnormal fetal cardiac axis is closely related to fetal CHD and has certain value in CHD diagnosis.
