经颅直流电刺激治疗轻中度老年抑郁症的临床观察

Clinical observation of transcranial direct current stimulation in the treatment of mild to moderate senile depression

摘要: 目的 探讨经颅直流电刺激(transcranial direct current stimulation,tDCS)治疗轻中度老年抑郁症的临床疗效。方法 选取2018年1月至2018年12月首都医科大学附属北京友谊医院康复医学科就诊的轻中度老年抑郁症患者40例,按随机数字表法分为观察组和对照组(各20例)。观察组采用tDCS,对照组采用相同的刺激部位和时间的伪刺激。分别于治疗前和治疗4周后采用24项版汉密尔顿抑郁量表(Hamilton Depression Scale,HAMD)、老年抑郁量表(Geriatric Depression Scale,GDS)、蒙特利尔认知评估量表(Montreal Cognitive Assessment,MoCA)评价两组患者抑郁及认知功能改善情况。结果 两组患者治疗前后HAMD、GDS变化的组间差异有统计学意义,观察组明显优于对照组(P<0.05),MoCA评分差异无统计学意义(P>0.05)。结论 tDCS可显著改善轻中度老年抑郁症患者的抑郁症状,值得临床推广。

Abstract: Objective To investigate the clinical efficacy of transcranial direct current stimulation (tDCS) in the treatment of mild to moderate senile depression.Methods From January 2018 to December 2018, forty patients with mild to moderate senile depression were selected from the Department of Rehabilitation Medicine, Beijing Friendship Hospital, Capital Medical University. They were randomly divided into observation group and control group (20 cases each). The observation group was treated with tDCS and the control group was treated with pseudo-stimulation of the same stimulation site and time. The 24-item Hamilton Depression Scale (HAMD), the Geriatric Depression Scale (GDS) and Montreal Cognitive Assessment Scale (MoCA) were used to evaluate the depression and cognitive improvement of the two groups before and after treatment.Results There was significant difference in the changes of HAMD and GDS between the two groups before and after treatment. The level of observation group was significantly better than that of the control group (P<0.05), and there was no significant difference in MOCA score (P> 0.05).Conclusion tDCS can significantly improve the depressive symptoms of patients with mild to moderate senile depression, which is worthy of clinical promotion.
