脑梗死与心肌梗死患者血脂、血尿酸、纤维蛋白原和血小板参数的变化及其意义

Significance of the Changes in Blood Lipid, Serum Uric Acid, Fibrinogen and Platelet Levels in Patients with Cerebral Infarction and Myocardial Infarction

摘要: 目的 探讨脑梗死与心肌梗死患者血脂、血尿酸、纤维蛋白原和血小板参数的变化及意义。方法 将206例患者按病因分为心肌梗死组(69例)、脑梗死组(72例)和脑梗死合并心肌梗死组(65例);76例健康人为正常对照组。分别检测三酰甘油(TG)、总胆固醇(TCH)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、载脂蛋白A1(APOA1)、载脂蛋白B(APOB)、血尿酸(UA)、纤维蛋白原(Fib)、血小板压积(PCT)、血小板平均体积(MPV)和血小板体积分布宽度(PDW)的水平。结果 脑梗死组、心肌梗死组患者的UA、Fib水较正常对照组升高,差异有统计学意义(P<0.01,P<0.05);MPV、LDL-C水平较正常对照组升高(P<0.05),且脑梗死和心肌梗死对UA、Fib、MPV、LDL-C指标的改变无交互效应(P>0.05)。脑梗死组和心肌梗死组患者的PDW水平较正常对照组升高、PCT水平较正常对照组降低,差异有统计学意义(P<0.05);TG、TCH和APOB的水平较正常对照组升高,差异有统计学意义(P<0.05);HDL-C和APOA1水平较正常对照组降低,差异有统计学意义(P<0.01),且脑梗死和心肌梗死对PDW、PCT的改变有交互效应(P<0.01)、对TG、TCH、APOB、APOA1、HDL-C的改变有交互效应(P<0.05)。结论 检测脑梗死与心肌梗死患者血脂、血尿酸、纤维蛋白原和血小板参数的变化,对判断病情及预后均有一定的意义。

Abstract: Objective To investigate the changes in blood lipid, serum uric acid, fibrinogen, platelet level in patients with cerebral infarction and myocardial infarction. Methods A total of 206 patients were divided into 3 groups: myocardial infarction group(n=69), cerebral infarction group(n=72), cerebral infarction complicated with myocardial infarction group(n=65), and 76 healthy persons were enrolled into control group. Triglyceride(TG), total cholesterol(TCH), high density lipoprotein cholesterol(HDL-C), low density lipoprotein cholesterol(LDL-C ), apolipoprotein A1 (Apo-A1), apolipoprotein B(ApoB), serum uric acid(UA), fibrinogen(Fib), plateletcrit(PCT), mean platelet volume(MPV) and platelet volume distribution width(PDW) were examined for all subjects. ResultsIn cerebral infarction group and myocardial infarction group, the UA, Fib level was higher than that of control group(P<0.01, P<0.05); the MPV, LDL-C level was higher than that of control group(P<0.05), and cerebral infarction and myocardial infarction had no interactive effect on the changes of UA, Fib, MPV, and LDL-C. The PDW level in cerebral infarction group and myocardial infarction group was higher than that of control group, PCT level was lower than that of control group(P<0.05); TG, TCH and APOB were higher than those of control group(P<0.05); HDL and APOA1 level were lower than that of control group(P<0.01). Cerebral infarction and myocardial infarction had interactive effects on the changes in PDW and PCT(P<0.01) and on TG, TCH, APOB, APOA1, and HDL-C(P<0.05). Conclusion Monitoring of blood lipid, serum uric acid, fibrinogen, and platelet levels is helpful in diagnosis and predicting the prognosis in patients with cerebral infarction and myocardial infarction.
