麝香保心丸对心肌梗死后心力衰竭大鼠心脏α1及β肾上腺素能受体表达的影响

Effects of Shexiangbaoxin Pills Therapy on the Expression of Cardiac α1 and β Adrenergic Receptors in Rats with Heart Failure Induced by Myocardial Infarction

摘要: 目的 探讨麝香保心丸对心肌梗死后心力衰竭大鼠非梗死区左心室组织α1及β肾上腺素能受体(adrenergic receptor,AR)mRNA表达水平的影响。方法 结扎大鼠左冠状动脉前降支制作心肌梗死后心力衰竭模型,54只Wistar大鼠随机分为正常对照组(A组)、假手术组(B组)、心力衰竭模型对照组(C组)、阳性药物对照组(D组)、麝香保心丸小剂量组(E组)和麝香保心丸大剂量组(F组),采用灌胃法给药。超声心动图检测用药前后大鼠心脏功能,半定量反转录聚合酶链反应(semiquantitative reverse transcription polymerase chain reaction,RT-PCR)法检测大鼠非梗死区左心室组织β1-AR、β2-AR、β3-AR、α1A-AR、α1B-AR、α1D-AR的mRNA表达水平。结果 ①用药前,A组与B组比较,大鼠左室射血分数(left ventricular ejection fraction,LVEF)组间差异无统计学意义(P>0.05);与B组比较,C、D、E、F组大鼠LVEF明显降低(P<0.01)。用药后,与C组比较,F组LVEF显著改善(P<0.05),D组、E组LVEF有升高趋势,但差异无统计学意义。②A组与B组用药后比较,各受体水平差异无统计学意义(P>0.05)。与B组比较,C组大鼠β1-AR、α1B-AR水平明显降低(P<0.01和P<0.05),α1A-AR、β3-AR水平明显升高(P<0.01),β2-AR、α1D-AR水平差异无统计学意义(P>0.05)。与C组相比,D、E、F3组β1-AR、α1B-AR水平均显著升高(P<0.05或P<0.01),α1A-AR水平均显著降低(P<0.05),β2-AR、α1D-AR水平未见明显改变(P>0.05)。D、F组β3-AR水平均显著降低(P<0.01)。结论 应用麝香保心丸治疗升高了LVEF,改善了心肌梗死后心力衰竭大鼠的心脏功能,由于心肌血供改善,调节了α1-AR和β-AR各亚型的表达水平,心力衰竭时下调的β1-AR、α1B-AR水平开始上调,上调的α1A-AR、β3-AR水平下调,从而对慢性心力衰竭起到了有益的治疗作用。同时,与小剂量麝香保心丸相比,大剂量疗效更为显著。

Abstract: Objective To investigate the effect of Shexiangbaoxin Pills(SXBXP) on the messenger RNA(mRNA) levels of α1-adrenergic receptor(AR) and β-AR expressed in non-infarcted left ventricular tissue from rats with chronic heart failure(CHF) induced by myocardial infarction.Methods Models of CHF established by anterior descending coronary artery ligature.Fifty-four Wistar rats were randomly divided into five groups:control group(group A),sham operation group(group B),CHF model group(group C),positive medicine group(group D),low dose SXBXP group(group E) and high dose SXBXP group(group F).Heart function was determined by echocardiography,mRNA expression levels of β1-AR,β2-AR,β3-AR,α1A-AR,α1B-AR,α1D-AR in non-infarcted left ventricular tissue were measured by semiquantitative reverse transcription polymerase chain reaction(RT-PCR).Results ① Before therapy,there was no significant difference between groups A and B.Compared with group B,the left ventricle ejection faction(LVEF) of groups C,D,E and F were significantly decreased(P<0.01).After therapy,compared with group C,LVEF of group F was significantly improved(P<0.05).LVEF of groups D and E increased even no significant difference was detected compared with group C.② After therapy,no significant change was seen in expression of any adrenergic receptor in groups A and B(P>0.05).Compared with group B,β1-AR and α1B-AR expression in group C markedly decreased(P<0.01 and P<0.05),expression of α1A-AR and β3-AR exhibited a remarkable increase(P<0.01),while there was no significant difference in β2-AR and α1D-AR expression between groups B and C(P>0.05).Compared with group C,levels of β1-AR and α1B-AR significantly increased(P<0.05 or P<0.01),α1A-AR markedly declined in groups D,E and F(P<0.05),β2-AR and α1D-AR expression remained unchanged in these groups(P>0.05).β3-AR level significantly declined in both groups D and F(P<0.01) compared with group C.Conclusion Heart function was improved and levels of LVEF was increased when SXBXP was used to treat rats with CHF induced by myocardial infarction.The action of SXBXP in CHF may be related to better blood supply to myocardium and regulation of α1-AR and β-AR expression towards normal,including the upregulation of β1-AR and α1B-AR and downregulation of α1A-AR and β3-AR.This observation supports the hypothesis that SXBXP may improve CHF,especially when it is applied at large dose.
