人格解体障碍患者社会功能、生活质量及满意度的状况及其相关性分析

Study on quality and satisfaction of life and the social function in patients with depersonalization disorder

摘要: 目的 探讨人格解体障碍患者的社会功能及生活质量水平与满意度。方法 对38例剑桥人格解体量表评分≥ 70分,且符合精神障碍诊断标准与统计手册第5版(Diagnostic and Statistical Manual of Mental Disorders,Fifth Edition,DSM-V)中诊断标准的人格解体障碍患者,采用功能大体评定量表(Global Assessment Function,GAF)、简明幸福与生活质量满意度问卷(Short-Form Version of the Quality of Life Enjoyment and Satisfaction Questionnaire,Q-LES-Q-SF)和世界卫生组织生活质量测定量表(World Health Organization Quality of Life Brief Questionnaire,WHOQOL-BREF)问卷评估受试者的社会功能和生活质量与满意度。结果 人格解体障碍患者在社会功能、生活质量的生活领域(physiology,PHYS)、心理领域(psychology,PSYCH)、社会关系领域(sociology,SOCIL)、环境领域(environment,ENVIR)和Q-LES-Q-SF总分及满意度均显著低于健康对照者。人格解体障碍组GAF、PHYS、PSYCH、SOCIL、ENVIR得分及Q-LES-Q-SF总分分别为(61.21±5.99)、(11.17±1.87)、(10.67±1.78)、(10.53±2.90)、(12.00±2.61)、(39.26±7.93),对照组分别为(85.88±2.81)、(12.96±1.96)、(13.33±2.32)、(14.50±2.64)、(13.45±2.18)、(50.88±8.38),两组相比差异均有统计学意义(P均<0.01);剑桥人格解体量表总分与心理领域、社会关系领域、环境领域和总的生活质量以及满意度均有相关性(P<0.05)。伴有焦虑症状的人格解体障碍患者的社会功能显著低于单纯人格解体患者[(57.67±5.69)vs(62.85±5.48),P<0.05]。结论 人格解体障碍患者的社会功能、生活质量及生活满意度较健康人差。它不仅表现在生理领域,更涉及患者的情感体验、社会功能及精神健康层面,使患者的生活质量和生活幸福感也随之下降。

Abstract: Objective To Explore the quality and satisfaction of the life and social function in patients with depersonalization disorder(DPD). Methods The total thirty-eight patients with depersonalization disorder who were diagnosed with the diagnostic criteria of the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-V), and the Cambridge Depersonalization Scale(CDS) of the patients scored ≥ 70. Forty normal persons were set as the control. All the patients and controls were evaluated with the Global Assessment Function (GAF), the Short-Form Version of the Quality of Life Enjoyment and Satisfaction Questionnaire(Q-LES-Q-SF) and the World Health Organization Quality of Life Brief questionnaire (WHOQOL-BREF) to assessed quality and satisfaction of the life and social function. Results Compared to the control group, the total scores of GAF and WHOQOL-BREF in the patients were significantly lower on the quality of the life(PHYS), psychological field (PSYCH), social relations field (SOCIL), environmental field (ENVIR), and the total scores of Q-LES-Q-SF. The score on GAF in DPD was 61.21±5.99, vs 85.88±2.81 in normal group. The score on PHYS in DPD was 11.17±1.87, vs 12.96±1.96 in normal group. The score on PSYCH in DPD was 10.67±1.78, vs 13.33±2.32 in normal group. The score on SOCIL in DPD was 10.53±2.90, vs 14.50±2.64 in normal group. The score on ENVIR in DPD was 12.00±2.61, vs.13.45±2.18 in normal group. The total score on Q-LES-Q-SF in DPD was 39.26±7.93, vs 50.88±8.38 in normal group. The total score of Cambridge Depersonalization Scale was correlated with psychological field, social relationship field, environment field, total quality of life and satisfaction (P<0.05). Compare to simple DPD, the total scores of GAF were lower in DPD with anxiety. The score on GAF in simple DPD was 57.67±5.69 vs 62.85±5.48 in DPD with anxiety(P<0.05). Conclusion The quality and satisfaction of life and social function of DPD was significantly worse than that of normal controls, regarding not only in the physiological aspect but also to the patient's emotional experience, social function and mental health level.
