自固定补片与钉合固定补片在巨大腹股沟疝行腹腔镜经腹腹膜前疝修补术中的对比研究

摘要/Abstract

摘要： 目的对比分析腹腔镜经腹腹膜前疝修补术（transabdominal preperitoneal，TAPP）手术中使用自固定补片与钉合固定补片治疗巨大腹股沟疝的临床效果。方法回顾性分析2014年1月至2016年1月首都医科大学附属北京朝阳医院京西院区疝和腹壁外科收治的共80例行TAPP手术的巨大腹股沟疝患者的临床资料，其中42例患者应用自固定补片（观察组），38例应用钉合方法固定普通补片（对照组），对比2组间手术时间、术后住院时间、住院费用、术后视觉模拟评分（visual analogue scale，VAS）、复发等术后合并症等指标。结果 2组均顺利完成手术，2组患者手术时间及复发等术后合并症差异均无统计学意义（P>0.05），术后住院时间及住院费用差异有统计学意义（P<0.05）。术后随访1年，2组间VAS差异有统计学意义（P<0.05）。结论应用自固定补片可降低巨大腹股沟疝患者的住院费用及术后住院时间，不会增加手术时间及术后疝复发等合并症的风险，且具有术后慢性疼痛轻的优点，具有较高的临床应用价值。

Abstract: Objective To analyze the self-gripping mesh and mesh fixed with tacks in laparoscopic transabdominal preperitoneal (TAPP) herniorrhaphy for giant inguinal hernia.Methods A total of 80 patients of giant inguinal hernia who were subjected to TAPP from Jan.2014 to Jan.2016 were studied. All patients were divided into two groups, including self-gripping mesh group (observation group, n=42) and mesh fixed with tacks group (control group, n=38). The operation time, hospital stay after operation, hospitalization cost, complications and visual analogue scale (VAS) after operation were observed. Results All the operations were successfully completed. There was no significant difference in the average operation time and complications, and comparing with control group, the self-gripping mesh group cost less money and caused less pain, the difference was significant (P<0.05).Conclusion Use of self-gripping mesh will decrease hospitalization cost and post hospital stay, and does not increase the risk of complications such as recurrence and it's an effectivemeasure for reducingpostoperative pain, which is worth to be popularized.
