Abstract: The lower one-third of the leg consists of a tight skin directly cove-ring bone and musculotendinous stuctures. Soft tissue defects in this area are difficult to repair. However the method of using bipedicle gastro-cnemius musculocutaneous flap to repir soft tissue defect on the lower one-third of the leg is good. The reported 4 cases of open fracture of the leg with soft tissue defect had been treated with this method and had obtained satisfactory results. The plenty circulation from the proxi-mal end of the flap allows the distal pedicle to become narrow and therefore be ahle to advance without jeopardizing the blood supply. This paper discussed the principles of designing the flap and its clinical ap-plication.
