Abstract:

Objective To investigate the relationship between admission blood sugar level(BSL) and the severity of injury in terms of injury severity score(ISS) in pediatric trauma patients, and try to find an objective index to estimate the prognosis of injured children. Methods A retrospective study was performed.Patients who were admitted to our hospital within 24 h after trauma were enrolled. Those who had been pre-diagnosed diabetic mellitus, been on corticosteroids treatment,or already been given dextrose infusion before BSL investigation would be excluded from the study.Data collected include time to obtain glucose measurement, admission blood sugar level, whether ICU care was required or not,and mortality.ISS was calculated according to Baker ISS.Patients were classified into three groups according admission BSL.Results A total of 163 patients were recruited.A significant direct correlation between admission BSL and ISS was found(r=0.566,P<0.01).ISS of the severe hyperglycemia group(≥11.0 mmol/L) was significantly higher than other two groups(P<0.01).Conclusion The admission blood sugar level had a significant and positive correlation with ISS in pediatric trauma patients,it may be used as a simple index,at least a numerical reference, indicating the severity of injury.Patients with admission BSL ≥11.0 mmol/L should be monitored closely.
