    One century has passed away since American doctor Ignatz L.Nascher put forward the term "Geriatrics".Today geriatric medicine or geriatrics has become one of the newest medical subjects through persistent efforts of gerontologist clinics and scientific research works in developed countries.Geriatrician has been recognized and the health service system for the elderly has been established.The geriatric research focuses on elderly patients but not only on diseases.The management and assessment are the two cores in geriatrics study,the purpose of which is not only treating disease but also preventing and protecting patient's body function.The evidence-based medicine has shown that the geriatric care about case management,multidisciplinary team work,health assessment,geriatric rehabilitation,long term care and end of life care can decrease disability and promote longevity,as well as control medical care expense and increase satisfaction of patients and family numbers.

    Objective By analyzing the status and rehabilitation demands of disabled elderly persons in China,this study intends to give suggestions to strengthen rehabilitation service and resolve the rehabilitation problems of disabled elderly persons.Methods Descriptive and comparative analyses were conducted to analyze the characteristics of the disabled elderly in China,the main factors leading to disability,and the differences between supply and demand of rehabilitation according to the data of the 2006 National Sample Survey on Disability.Results The number of female elderly people with disabilities was higher than that of the male,and the proportion of disability increased with ages.More than three quarters of disabled elderly persons lived in rural areas.Hearing disability,physical disability and visual disability were the main types of disability,which accounted for more than ninety percent.Among the factors leading to disability,disease was the most important one.The disabled elderly had different rehabilitation demands in terms of gender,age and types of disability.The main demands were medical services and supplementary instruments and trainings.However,most of the rehabilitation demands hadn't been met,especially those of the female and the oldest.Among the rehabilitation demands supplementary,instruments had the largest gap between its supply and demand,followed by the trainings and services of rehabilitation.Medical service and assistance could meet the demand to the largest extent,but only accounted for about fifty percent.Conclusion China needs to promote healthy ageing,prevent diseases,reform the expenditure of personal and public finance,and at the same time,pay more attention to the construction of medical personnel in order to meet the demands of rehabilitation of disabled elderly persons.

    The platelet membrane glycoprotein plays a key role in the adhesion,aggregation and release of platelets,and is the specific marker for platelet activation.Many studies have shown that the activity of the platelet membrane glycoprotein,especially CD62P,CD63 and PAC-1,can reflect the pathogenesis and development of coronary heart disease sensitively and specifically,and its expression can be influenced by the pathological conditions.Blood-stasis syndrome is one of the most common clinical syndromes.Patients having different diseases with blood-stasis syndrome do have platelet activation,but in terms of the platelet activity,there is no difference among them.Measurement of the activated platelet membrane glycoprotein not only provides help to diagnose acute myocardial infarction sooner and estimate the state of the illness,but also can be served as one of the indicators of micro-dialectical discrimination of illness.

    Objective To survey the regularities of dopamine neurons in substantia nigra(SN) of middle brain with age increasing and further provide objective evidence for revealing lesion of substantia nigra.Methods Nineteen normal individuals were divided into young(17 to 35 years old),middle-aged(40 to 59 years old),and aged(60 to 84 years old) groups.The age-related changes of neuromelanin(NM) containing and tyrosine hydroxylase(TH) immunoreactive cell number of SN were analyzed.Results Stereological counting based on Nissl stain and TH immunoreactivity revealed that,both Nissl stained and neuromelanin containing neurons decreased in the process of ageing,while the decrease of both TH immunoreactive and NM containing cell number was the most significant changes.Conclusion NM may participate in ageing influenced degeneration and loss of dopaminergic neurons.And that may serve as the foundation of neurodegenerative disease.
    The Conversion Rates from Mild Cognitive Impairment to Alzheimer Disease in Elderly Veterans: A Population-based Five-year Follow-up Study

    Objective To evaluate the conversion rates from mild cognitive impairment(MCI) to Alzheimer disease(AD) in elderly veterans during an average of 5 years follow-up.Methods This study was a prospective cohort study based on diagnosis.The study subjects were derived from a prevalence investigation of MCI in Shijiazhuang,China,in the year 2001.A total of 216 MCI subjects(mean age,74.2 years) and other 2 302 normal cognitive(NC) subjects(mean age,73.9 years) were followed up for an average of 5 years.The diagnosis of dementia and AD were assessed by neurologists using the DSM-IV and NINCDS-ADRDA criteria.The main outcome measures were the annual conversion rates of MCI and NC subjects to AD,the relative risks(RR) of MCI and NC developed AD and 95% confidence interval(95% CI).The conversion rates were calculated using the person-year method.Results After 5 years,the mortality of MCI subjects was 7.9%,and for NC subjects,3.4% respectively(P=0.001).After 3 years,the conversion rate of MCI subjects to AD was 16.8%,and after 5 years,the conversion rate of MCI subjects to AD was 33.7%.The annual conversion rate of MCI subjects to AD was 6.7(95% CI=5.4~8.0) per 100 person-year,and for NC subjects,0.8(95% CI=0.6~1.0) per 100 person-year respectively,but no difference was found between male and female(P=0.62).The conversion rates of MCI subjects to AD were decreased with the elevation in degree of education(P=0.03).The relative risk of MCI subjects to AD was 8.1(95% CI=6.1~10.6,P<0.0001) times as compared with NC subjects.Conclusion The conversion rates of MCI subjects to AD in elderly veterans is very higher than the normal cognitive subjects.There are sufficient data to recommend the evaluation and clinical monitoring of the elderly persons with mild cognitive impairment due to their increased risk for developing AD.

    Objective To explore the relation between the P141L polymorphism of urokinase plasminogen activator gene(PLAU) and Late-onset Alzheimer's disease(LOAD) in Hans.Methods A total of 67 LOAD patients based on the diagnosis standard of DSM-Ⅳ and NINCDS-ADRDA and 71 subjects of the same age and the same sex construction as those in the case group were enrolled in this case-control study.The polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP) technique was used in detecting the polymorphism of the PLAU.Results The genotype frequencies distribution of both LOAD group and control group was in line with the Hardy-Weinberg law;No significant difference was found between the case group and the control group in terms of the genotype distribution(χ2= 0.223 8,P=0.636 1).No significant difference was found between the two groups in the distribution of allele(χ2= 0.223 8,P=0.636 1).Conclusion No relation was found between polymorphisms of PLAU and the LOAD disease in Hans.

    Objective To investigate the effects of chronic diseases on the incidence of depression symptom in the elderly between the urban and rural areas.Methods The sample population consisted of 2 506 elderly people was sampled from urban and rural communities in Beijing with well-established statistical sampling conditions and techniques such as clustering,stratification and random selection.Data were collected by trained staffs using standard survey instruments in years 2000 and 2004.Results The incidence of depression symptom in rural area was higher than that in urban area,among the elderly adults who had variety chronic diseases.In the rural area,the incidence of depression among people who had cardiovascular,respiratory,cerebrovascular or digestive system diseases was significantly higher than that of the people who didn't have corresponding disease.In the urban,the incidence of depression in the elderly who had hearing disturbance was also significantly higher than those who had normal hearing.Conclusion It is important to develop health education and spread the health knowledge in the elderly,for improving medical care assurance,prolonging the life of the elderly and elevating their qualities of life especially in the rural areas.

    Objective To evaluate the therapeutic value of about the stroke unit of traditional Chinese Medicine.Methods Randomized controlled observations had been carried out.The main observation indexes included mortality,infection rate,Barthel Index(BI),National Institutes of Health Stroke Scale(NIHSS).Results There was no significant statistical difference in risk factor,diagnosis NIHSS and BI between two groups before treatment(P>0.05).There was significant statistical difference in NIHSS and BI between two groups after treatment(P<0.05).Conclusion The stroke unit of traditional Chinese is superior to the stroke unit.

    Objective To evaluate the implementation effect of clinical pathway(CP) in intertrochanteric hip fractures in the elderly population undergoing DHS for internal fixation.Methods After consulting the information of the hospitalized patients with intertrochanteric hip fractures after internal fixation with dynamic hip screw(DHS) from October 2002 to October 2006,we analyzed the implementation effect of CP using the average duration of hospitalization and the hospital expenditure for the patients as the evaluating indexes.Results There was significant difference in terms of the average duration of hospitalization and expenditure between the CP groups and the traditionally treating groups.Conclusion The method of clinical pathway can markedly decrease the duration of hospital stay and the expenditure for intertrochanteric hip fractures undergoing DHS for internal fixation.

    Objective To explore the characteristics of the elderly emotional disorders and countermeasures.Methods Two hundred cases of elderly anxiety and depression of the data were analyzed,relationships between elderly anxiety,depression and physical disease,gender,age,education,occupation,sleep and so on were discussed.Results The elderly anxiety and depression in the level of education at all levels of people occured,and low education were symptoms of lighter,higher education were symptoms of heavier.The patients of stroke,hypertension,coronary heart disease,COPD,cancer,sleep disorders and in patients with peptic ulcers were usually had emotional disorders.Male was more than female,elderly patients were more than younger patients.Conclusion The elderly anxiety,depression are related to physical disease,age,gender,education,occupation,sleeping disorder.Doctors should pay attention to identify the elderly emotional disorders,timely processing.

    The Effect of RNA Interference Silencing Cyclooxygenase-2 Gene on Proliferation and Apoptosis of Esophageal Squamous Carcinomas EC109 Cell

    Objective To study the relationship between cox-2 gene and the proliferation as well as apoptosis of esophageal squamous carcinoma EC109 cell at the gene level;also by studying the effects of aspirin(non-specific cooxygenase-2 inhibitor) on proliferation and apoptosis of esophageal squamous carcinoma EC109 cell,to investigate the mechanism of gene and medical therapies for esophageal squamous carcinoma.Methods The RNA interference method was used in this study.Short interfere RNA was synthesized targeting Cox-2 Gene.The expression of COX-2 gene in esophageal squamous cell carcinoma EC109 was specially inhibited by RNAi technique.The efficiency of RNAi technique was detected with fluorescence microscopy.The effect of RNAi technique was detected with Western blot.Cell growth was tested with MTT reduction assay.The cell apoptosis was detected with flow cytometry.EC109 growth was tested with MTT reduction assay.Results Compared with the control group,the expression of COX-2 gene in EC109 had a 40% decrease.The OD value of MTT was 0.85±0.05 in mother cell group,the OD value of MTT was 0.75±0.06 in negative control group labeld by FITC,the OD value of MTT was 0.48±0.05 in transfection group of COX-2 siRNA,the OD value of MTT was 0.28±0.04 in Aspinin combined with RNAi group.After RNAi,the effect of growth inhibition was increased in EC109 cell compared with the control groups.The growth inhibition of Aspinin combined with RNAi group was stronger than that of RNAi group.The rate of apoptosis was 1.35% in mother cell group,the rate of apoptosis was 1.72% in negative control group labeld by FITC,the rate of apoptosis was 4.33% in transfection group of COX-2 siRNA,the rate of apoptosis was 11.66% in Aspinin combined with RNAi group.After RNAi,the rate of apoptosis increased in EC109 cell compared with the control groups.The rate of apoptosis of aspinin conbined with RNAi group was stronger than that of RNAi group.Conclusion The RNAi techniques can specifically inhibit the expression of COX-2 gene.The cell proliferation has been suppressed and the cell apoptosis is stronger in EC109 after COX-2 gene has been specifically inhibited.The combined administration of aspirin and RNAi has a synergic effect on the tumor cell proliferation and apoptosis.

    Objective More and more researchers become focused on myocardial damages after acute brain injury are diverse.Characteristics of myocardial damages after acute brain injury are divere.The mechanism is not clear and there isn't affirmative method of prevention and treatment.It is of important clinical significance to study the mechanism,prevention and treatment to improve prognosis,decrease mortality and increase the number of heart donation.Objectives of this study are to investigate the process of myocardial damage after acute brain injury and to study the effects of 5-HT in plasma and myocardium on myocardial damage after acute brain injury in rat.The study can provide further insights into the relationship between the brain and the heart,and the prevention and treatment of myocardial injury after acute brain injury.Methods Acute brain injury rat model was induced with weight-drop technique.Electrocardiogram,cardiac function,serum CK-MB(heart-type creatine kinase isoenzyme) and morphologic alterations in myocardium were measured to indicate and diagnose the degree of myocardial damage.Electrocardiogram was recorded using twelve leads electro cardiograph.Alterations of cardiac function included ±dp/dtmax,LVDP(left ventricular developed pressure) and LVEDP(left ventricular diastolic end pressure).serum CK-MB was measured using enzyme reaction rate method.The morphologic alterations in myocardium were observed with light and electronic microscopes.5-HT in plasma and myocardium were detected by high performance liquid chromatography-electrical conductivity detector.Rats were divided into sham group and acute brain injury group in the experiment.Alterations were observed at 4 time point including 1 hour,6 hour,24 hour and 48 hour after acute brain injury.Results The cardiac abnormalities after acute brain injury had been shown as below.Incidence of abnormal ECG increased(P<0.05);Positive and negative dp/dtmax and LVDP decreased(P<0.05);LVEDP increased(P<0.05);CK-MB increased(P<0.05).Degeneration,necrosis and hemorrhage of myocardium were observed by light and electronic microscopes,which included increasing and swelling of mitochondria and disruption of cardiac muscle fiber.The most severe myocardial damage happened in 24 hours after ABI.Myocardial systolic dysfunction happened earlier than diastolic dysfunction.Plasma 5-HT increased significantly(P<0.05).Positive correlation was found between 5-HT in plasma and CK-MB(P<0.05).Conclusion Acute brain injury can lead to myocardial damage.5-HT may play an important role in the mechanism of myocardial damage after brain injury.

    Objective The interaction of mizolastine with bovine serum albumin(BSA) was investigated with quenching of BSA fluorescence by mizolastine.Methods Fluorescence spectroscopy was employed to study the Stern-Volmer quenching constants(Ksv) of BSA in the presence of mizolastine in pH 7.4 PBS.The CD and UV absorption spectra were used to investigate the effect of mizolastine on the conformation of BSA.Results The quenching constants(Ksv) and corresponding thermodynamic parameters(ΔH,ΔS and ΔG) were calculated from the spectroscopic data at different temperatures.Conclusion The Ksv suggests that the quenching of BSA fluorescence by mizolastine occurs through the static process.The ΔH,ΔS and ΔG obtained at different temperatures show that the binding of mizolastine to BSA is mainly driven by hydrophobic force.The addition of mizolastine leads to the conformational changes of BSA.

    Objective To investigate the characteristics of the tissue oxygenation changes in the MODS animal model.Methods Sixteen one-year-old male swines weighting was 20~25 kg were involved.Bacilius(17 million/kg) was injected into the right jugular vein,the tissue oxygenation-index(arterial oxygen content,CaO2,oxygen delivery,DO2,oxygen consumption,VO2,oxygen delivery index,DO2I,oxygen consumption index,VO2I,oxygen extraction rate,O2Ext) were determined at 1 h,3 h,5 h,7 h after injection.Results The study showed that,DO2,VO2 had no remarkably difference after 1,3,5 hours of Colon bacillus injection,after 7 hours have no remarkably difference compared with that was before making mode(P<0.01),but DO2I,VO2I had very remarkably difference compared with that was before making mode(P<0.01),after 1,3,5,7hours of Colon bacillus injection.After 5,7 hours injected Colon bacillus,the extract rate of oxygen(O2Ext) had remarkable difference(P<0.05).With linear correlation analysis for DO2I-VO2I,it was found DO2I was remarkably linear correlated with VO2I(r=0.784,P<0.01),therefore,they had the dependent relation.Conclusion These findings provide the experimental basis for the clinical study of MODS.
    The Changes in Coagulation-fibrinolysis Systems in Rabbits with Steroid-induced Avascular Necrosis of the Femoral Head

    Objective To observe the changes in parameters of coagulation-fibrinolysis systems in rabbits with steroid-induced avascular osteonecrosis of femoral head(SANFH) and to investigate the pathogenesis of SANFH.Methods Experimental animal and grouping:Male adult New Zealand white rabbits,ranging in weight from 2.5 to 3.5 kg,were obtained from experimental center of Capital Medical University and randomly divided into seven groups including Group N(normal group);Groups M24 h,M48 h,M1,M2,M4,M6(model groups).Eight rabbits for each group.Animal models of steroid-induced avascular osteonecrosis of the femoral head were established according to Yamamoto T et al's method.Animals in model group received lipopolysaccharide(LPS) 20 μg/kg intravenously 2 times at an interval of 24 hours,methylprednisolone 20 mg/kg injected were intramuscularly into the gluteus medius 3 times at intervals of 24 hours after the second injection of LPS.The plasma levels of antithrombin-Ⅲ(AT-Ⅲ),thrombomodulin(TM),tissue-type plasminogen activator(tPA) and plasminogen activator inhibitor-1(PAI-1) were measured in each group.Results 1) Coagulation system:AT-Ⅲ:Compare with the normal group,the plasma levels of AT-Ⅲ in the model group were significantly higher at 24 hours after the first injection of LPS(P<0.05),then began to decrease at 48 hours,most obviously at 1 week(P<0.05);TM:Compare with the normal group,the plasma levels of TM in the model group began to increase at 24 hours after the first injection of LPS(P<0.01),most obviously at 1 week(P<0.01).2) Fibrinolysis system:tPA:The plasma levels of tPA in the model group were significantly lower than those in the normal group from 24 hours to 2 weeks after the first injection of LPS(P<0.01);PAI-1:The plasma levels of PAI-1 in the model group were significantly higher than those in the model group from 24 hours to 2 weeks after the first injection of LPS(P<0.01).Conclusion Marked dysfunction of coagulation-fibrinolysis systems has been found in the model animals,which may be one of the pathogeneses of SANFH.
    Changes in Tubular-interstitial Pathomorphology and Expression of TGF-β1,HGF and CTGF in a Rat Model of Type 2 Diabetic Nephropathy

    Objective To develop a rat model of type 2 diabetic nephropathy and to investigate changes in tubular-interstitial pathomorphology and expression of TGF-β1,HGF and CTGF in early-stage diabetic nephropathy.Methods Forty-four eight-week-old male Wistar rats were fed a normal or high-energy(46% of total calories from fat) diet for 20 weeks,and at week 6 rats received once intraperitoneal injection of citric acid buffer or STZ(30 mg/kg).In the eighth week,two groups underwent intraperitoneal injection glucose tolerance test(IPGTT).Body weight,fasting blood glucose,insulin,24 hour urine albumin excretion and creatinine clearance rate were monitored after the feeding programme.The tubular structural changes were examined with HE staining and periodic acid-silver methenamine staining at week 14 and week 20.The protein expression of TGF-β1,HGF and CTGF in tubular-interstitial were determined by immunohistochemistry at week 14 and week 20.Results 1) The rats fed high-energy markedly showed obesity,hyperglycaemia,hyperinsulinemia,insulin resistance and albuminuria.2) There were severe structural alterations of hepatic tissue,pancreas and tubular-interstitium in rats fed high-energy diet.3) The protein expression of TGF-β1 and HGF increased significantly in rats fed high-fat(P<0.05),while CTGF had no significantly changed.Conclusion 1) Tubular-interstitial pathomorphology in the rats of the early-stage diabetic nephropathy have been fibrosis.2) The protein expression of TGF-β1,HGF and CTGF in tubular-interstitium are unbalanced,which may be associated with diabetic nephropathy progression.

    Objective To investigate the beneficial effect of hepatocyte growth factor(HGF) on diabetic nephropathy(DN),and to observe the effect of HGF on the degradation of extra-cellular matrix(ECM) and production of TGF-β1 in cultured rat glomerular mesangial cells under high glucose condition.Methods MTT colorimetry was used to detect the proliferation of rat mesangial cells cultured under various concentrations of glucose and HGF.Then the cultured mesangial cells were divided into three groups as following:1) normal control group:the concentration of glucose in medium was 5 mmol/L;2) high glucose group:the concentration of glucose in medium was 30 mmol/L;3) HGF treatment group:the concentration of glucose in medium was 30 mmol/L and concentration of HGF is 50 μg/L.These cells were cultured for 48 hours.Then the supernatant content of TIMP-1,MMP-9,TGF-β1,HGF and Ⅳ collagen were measured by ELISA and the level of TIMP-1,MMP-9,TGF-β1,HGFmRNA were measured by RT-PCR.Results 1) HGF suppressed the proliferation induced by high glucose in cultured mesangial cells.2) High glucose concentration could stimulate the secretion of TIMP-1,TGF-β1,Ⅳcollagen and reduce the secretion of MMP-9.Compared to the high glucose group,HGF could suppress the secretion of TIMP-1,TGF-β1,and Ⅳ collagen but had little effect on MMP-9.3) Compared to high glucose group,the expression of TIMP-1mRNA and TGF-β1mRNA were significantly decreased in HGF treated group,while the expression of MMP-9mRNA and HGFmRNA were significantly upregulated.Conclusion The results suggest that HGF can protect the cultured mesangial cells from injury of high glucose concentration.This protective effect of HGF is exerted through the following mechanisms:HGF inhibit the proliferation of the mesangial cells under high glucose condition;increases ECM degradation by modulation of the balance between MMP-9 and TIMP-1;suppresses the secretion of TGF-β1 and upregulates the expression of HGF to rebuild the balance between TGF-β1 and HGF.

    Objective The purposes of this study are to evaluate the mechanism of delivering insulin with enhancer and additives on the in vitro cultured monolayer system of HIOEC(the human immortalized oral epithelial cell) lines.Methods The HIOECs formed continuous monolayers with polycarbonate filters of Transwell plate served to evaluate the transport of insulin.The transport studies were carried out at different conditions,with different penetration enhancers(HP-β-CD,chitosan,polyoxyethylene lauryl ether,egg lecithin,or oleic acid),and different additives(poly-L-lysine,colchicine).Results Under different conditions,the transport of insulin was not direction-,temperature-,and concentration-dependent.Conclusion The enhancing effects might be due to one or multiple factors:increasing the mucosal lipid fluidity,directly loosing the tight junction,and changing the steric conformation of protein.

    Objective To overcome the difficulties of delineating hippocampal boundaries in an effective way and benefit the related researches and applications by quantitative information from MR images,the paper presents a new method of computer-aided segmentation.Methods All possible edges at the region of interest were extracted by de-nosing edge detection methods with threshold zero,and they were highlighted.Compared with the original image,the edges which belong to the boundaries of hippocampus were identified and others were removed by a tool.The tool was the computer program developed for aiding to remove edges and modified the boundaries of hippocampal formation.Multiple edges could be removed by marking the two diagonal points of the region at which edges located.With the help of the tool,the magnified boundaries could be further improved by adding the missed pixels and deleting the redundant points,which guarantee the accuracy of the boundaries.Results Most parts of hippocampal boundaries could be well detected by Sobel operators.This made the proposed method more reproducible than manual segmentation.It took lower cost to remove undesired edges by the tool than to delineate hippocampal boundaries by manual tracing.With the guide of the boundaries detected,it was easier to locate the missing reqions of boundaries.Adjusting few pixels at boundaries made it possible to achieve the same accuracy as manual segmentation.Conclusion Although the proposed method cannot free us from manual operation,it is simple and practical.Before reliable automated approaches are fully available,the technique may be a better alternative.

    Objective To evaluate the effects of fleabane in the treatment of acute cerebral infarction(ACI).Methods We studies the patients with acute cerebral infarction who were admitted to Beijing Tiantan Hospital from January 2005 to January 2008 and meet the criteria of study inclusion.The patients with ACI were divided into two groups randomly:With the base of primary treatment of cerebral infarction,the treatment group was given the other served as the placebo control group.Before and after 15 days of treatment,traditional Chinese medicine symptom rating scale(SRS) of the two groups were evaluated respectively,by comparing the score changes in these two groups.Results The difference between the two groups was significant(P<0.05) by traditional Chinese medicine symptom rating scale and traditional Chinese medicine stroke curative effect evaluation.Conclusion The efficacy of fleabane is better than that of control group for treating ACI.

    Objective To analyze the clinical features,neuroimaging presentations,treatment and prognosis of skull mass-like plasma cell myeloma.Methods The clinical data of 5 patients with skull mass-like plasma cell myeloma,which were confirmed by histological examination and immunohistochemistry,were retrospectively analyzed.Preoperative radiographies included CT and /or MRI in all patients were obtained;all patients underwent direct surgery,and 4 patients received a course of postoperative radiotherapy,and /or chemotherapy.All patients were followed up from 6 to 120 months.Results The age of patients with skull mass-like plasma cell myeloma ranged from 36 to 66 years with an average of 51.8 years.The incidence was about 0.03% of intracranial tumors in our hospitalfor the same period.The initial characteristic symptoms and signs of these patients included skull mass,headache and cranial nerve palsy.CT features showed an expansile,lytic lesion involving the diploic space and the inner and outer tables of the skull,typically compressing adjacent brain;diffuse homogeneous enhancement after contrast material administration was generally seen,erosion of the full thickness of the skull was different from meningioma.MRI was extra axial and nearly isointense with gray matter on T2-weighted MR images,and diffusely enhanced after administration of contrast material.The tumors of 4 patients were totally removed,1 skull base tumor was partially removed.During operations the tumor often showed hypervascular presentation involving the skull.There were no operative mortality and serious complications in this series.During the follow-up,2 patient were dead,survival time was 120 and 2 months separately,3 were still alive,follow-up time was 118,12 and 6 months.Conclusion The imaging of skull mass-like plasma cell myeloma bore some similarities to meningioma.Erosion of the full thickness of the skull has differential value.Finding of systemic multiple lesions is essential for treatment and evaluation of prognosis.Controlling the bleeding during the operation is important.Post-operative radiotherapy and chemotherapy following can improve the patient's prognosis.

    Objective To investigate the Ki-67 and p16 protein expressions in the squamous intraepithelial lesions of the cervix.Methods S-P immunohistochemistry method was used to detect the expressions of Ki-67 and p16 in cervix samples.Results The positive rate of Ki-67 protein in the inflammatory lesions,CIN1,CIN2 and CIN3 were 50%,47%,55%,77%;The positive rate of p16 protein in the inflammatory lesions,CIN1,CIN2,CIN3 were 41%,59%,72%,91%.The expressions of Ki-67 and p16 protein were related to each other and correlated with the grading of CIN.Conclusion The expressions of Ki-67 and p16 are correlated with the grading of CIN,so they can be helpful in the diagnosis of lesions of the cervix.Especially they may facilitate the correct diagnosis in difficult cases such as the metaplastic lesions of old women;meanwhile,since the expressions of Ki-67 and p16 could be seen in the inflammtory lesions and the CIN1 lesions,attention should be paid to patients who had the strong positive staining of the two proteins.They should be followed up.In case the lesions show higher CIN grading(CIN2-3) with negative staining of the two proteins,attention should also be paid to avoid excessive treatment.

    Objective To investigate the clinical features of community acquired pneumonia(CAP) due to Legionella pneumophila(Lp) and non-pneumophila Legionella species(non-Lp).Methods Sputum samples from patients with clinically diagnosed legionella pneumonia according to manifestations and serum antibody results were detected for legionella DNA with a duplex polymerase chain reaction(DPCR)method.Patients with the coincident positive results of sputum DPCR and serum antibody were divided into Lp pneumonia group(42 cases) and non-Lp pneumonia group(18 cases).The clinical data of patients from these two groups were analyzed and compared.Results The diagnosis of pneumonia due to Lp or non-Lp was further confirmed since the results of DPCR were in accordance with that of serum antibody in the patients.Lp pneumonia often occurred in summer-autumn season and was more often found in middle-aged person in good health;whereas the majority of non-Lp pneumonia happened to the old with underlying diseases and seemed to have no seasonal preference no season.When compared with non-Lp pneumonia,the patients with Lp pneumonia showed wider pulmonary involvement,higher incidence of pleural effusion and ARDS,as well as a more significant symptom of systemic toxicity and extra-pulmonary involvement(P<0.05).Conclusion The clinical characteristics of CAP due to Lp are different from those due to non-Lp.Early treatment with macroazalides and fluoroquinolone is the most important way to improve the prognosis.

    Objective To analyze characteristics of onset time in patients with ST segment elevation myocardial infarction.Methods One thousand and twenty-three patients with STEMI were divided into different groups according to the timing of day,week and year when AMI occurred.The number and percentage of patients were observed in various subgroups.Uniformity was tested by a chi square test.Results 1) Onset was less in summer than that in other seasons.It was minimal in June(P<0.05).2) Three peaks of onset of AMI were observed in weekly distribution,they were Tuesday,Friday and Saturday.There was no significant difference between middle-aged people and old people.3) There were five peaks of onset among 24 hour in the day,they were 0~1,6~7,9~10,13~14 and 17~18 o'clock(P<0.05).Onsets were more frequent at 6~9,13~17 and 21~24 o'clock than that in other period of the day.Conclusion Onset of AMI is more frequently seen at certain times in a day,it may be related to physiological cycle and influenced by surrounding circumstances.

    Objective To investigate the risk factors related to the formation of the carotid atherosclerotic plaque so as to prevent and treat the cardiovascular or cerebrovascular diseases evidentially as early as possible.Methods A total of 192 inpatients admitted to the Medicine Department from July 2006 to July 2007 were included.They were all examined with the carotid colour Doppler flow imaging,and were grouped as plaque group(n=109) and non plaque group(n=83) according to the results of duplex color Doppler flow imaging.Patients of the two groups were compared statistically in the level of some possible risk factors including age,sex,drinking,smoking,hypertension,glycometabolism abnormality,blood glucose(BG),blood cholesterin(CHOL)and triglyceride(TC),high density lipoprotein(HDL)and low density lipoprotein(LDL).Logistic regression model was applied in the screening of risk factors.Results The mean age,mean total cholesterol,mean triglyceride,incidences of hypertension and abnormal glycometabolism were significantly higher in patients with carotid plaques than in patients without by single-factor analysis.No significant difference of sex,drinking,smoking,the mean value of low-density lipoprotein,high-density lipoprotein and glucose was found between two groups.Logistic multi-factor regression analysis showed that age(OR=1.10,P=0.002),hypertension(OR=2.81,P=0.031) and abnormal glycometabolism(OR=2.74,P=0.028) were involved in the regression equation statistically.Conclusion The formation of the carotid atherosclerotic plaque is interactional result of multiplicate factor.High total cholesterol and high triglyceride are the related risk factors of carotid plaque.Age,hypertension and abnormal glycometabolism are the possible independent risk factors for carotid plaque.

    Objective Dobutamine 99mTc-MIBI SPECT has been shown to enhance the detection of myocardial viability,this study evaluated its prognostic value in patients with coronary heart disease and left ventricular(LV) dysfunction after CABG.Methods Before CABG,38 patients(31 male,7 female;mean age was(66.5±7.0) years with LV dysfunction(42.7±12.5)% underwent the rest-dobutamine stressed 99mTc-MIBI SPECT.The rest 99mTc-MIBI SPECT follow up was performed 3 months later.Results(1) A post-revascularization improvement in ejection fraction(EF) ≥5% was defined as significant,patients were divided into tow groups:group A:LVEF increased significantly ≥5% in 19 patients at follow-up and group B:LVEF increased<5% in 19 patients.After CABG,LVEF in group A increased(P<0.001);end-diastolic volumes(EDV) and end-systolic volumes(ESV) decreased significantly(P<0.05);but LVEF,EDV and ESV had no change in group B.Three patients in group B had recurrent heart failure at follow-up.(2) According to receive-curve analysis,an increase in ejection fraction≥4.5% after dobutamine administration was the optimal cutoff value for predicting a significant postrevascularization improvement,with 76.9% sensitivity,64% specificity,and 68.4%accuracy.(3) A significant correlation was found between dobutamine and postrevascularization ejection fraction(r=0.83,P<0.001).The increase in ejection fraction during dobutamine administration is a good predictor of an improvement in ejection fraction after revascularization.Conclusion An increase in ejection fraction ≥4.5% during dobutamine 99mTc-MIBI SPECT was the optimal cutoff value for predicting a significant postrevascularization improvement.

    Objective The TNF-related apoptosis-inducing ligand is currently under evaluation as a possible(co-)therapeutic in cancer treatment,and it can notably induce the apoptosis of the tumor cells.To study the effect of TRAIL on leukemic cells of the patients,we have investigated the susceptibility of 13 acute leukemic cell samples from acute leukemic patients to TRAIL-induced apoptosis in vitro.Moreover,13 cell samples are coincubated with Ara-C in order to research the synergy between TRAIL and anti-cancer drug.Methods Thirteen leukemic cell samples were from acute leukemic patients who cared in Beijing Friendship Hospital from September 2002 to January 2003.The cell samples contained more than 60% leukemic cells based on morphology criteria.Every cell sample was separated into 4 groups:control group,TRAIL group,Ara-C group,TRAIL+Ara-C group.The cell samples were cultured 24 hours and the apoptotic cells were analyzed by flow cytometry after stained with Annexin Ⅴ and PI.Results Control group:the apoptotic cells were(10.22±7.48)%,TRAIL group:the apoptotic cells were(14.61±11.95)%,Ara-C group:(15.95±11.12)%,TRAIL+Ara-C group:(22.11±15.97)%.There were not significant statistically differences in three groups:control group and TRAIL group(P=0.35),TRAIL group and TRAIL+Ara-C group(P=0.12),Ara-C group and TRAIL+Ara-C group(P=0.19).There was just statistically difference between control group and TRAIL+Ara-C group(P=0.015).Furthermore,3 of 13(15%) cell samples revealed at least 10% TRAIL-susceptibility cells.Coincubated with Ara-C sentitized 2 initially TRAIL-resistant samples to TRAIL-induced apoptosis(>20% specific TRAIL-induced apoptotic cells).Conclusion 1) Only 23% leukemic cell samples are TRAIL sensitive cases;2) Coincubated with TRAIL and Ara-C augmented apoptosis in leukemic cells,including initially TRAIL-resistant samples.

    Objective To investigate the incidence and risk factors of parathyroid gland enlargement in patients on maintenance hemodialysis.Methods A total of one hundred and seventy-six cases with uremia on maintenance hemodialysis were studied in terms of parathyroid gland size and bone metabolism index,such as serum intact parathyroid hormone(iPTH),calcium,phosphorus and alkaline phosphatase(ALP) etc.Results 1) According to the result of serum iPTH,176 patients with uremia on maintenance hemodialysis were divided into three groups:high-PTH group(serum iPTH>300 ng/L),there were 84 patients(47.7%),normal-PTH group(serum iPTH 150~300 ng/L),there were 32 patients(18.2%) and low-PTH group(serum iPTH<150 ng/L),there were 60 patients(34.1%).2) Among the total of 176 patients,60 patients(34.1%) suffered from parathyroid gland enlargement:48(57.1%) patients in high-PTH group,4 patients(12.5%) in normal-PTH group and 8(13.3%) in low-PTH group.The incidence and degree of parathyroid gland enlargement in high-PTH group were significantly higher than those in normal-PTH group and low-PTH group.3) Results of the univariate analysis and multivariate logistic regression analysis showed that the dialysis time,the usage of active vitamin D,serum phosphorus,serum calcium,serum ALP and the enlargement of hemofiltration were influencing factors for parathyroid gland enlargement in patients on maintenance hemodialysis.The dialysis time,unregular use of active vitamin D,serum phosphorus and ALP were positively correlated;the frequency of hemofiltration and serum calcium were negatively correlated with parathyroid gland enlargement.Conclusion The incidence of parathyroid gland enlargement is high in patients on maintenance hemodialysis.The duration of dialysis,the level of serum iPTH and the use of active vitamin D play a positive relation to parathyroid gland enlargement in patients with uremia on maintenance hemodialysis.

    Objective The purpose of this study is to investigate the alterations of left ventricular function in patients with acute hypertensive cerebral hemorrhage by way of echocardiography and plasma BNP concentration determination and to shed light on the mechanism and to provide the basis for preventing and treating cardiac impairment caused by acute cerebral hemorrhage.Methods Thirty patients with acute hypertensive cerebral hemorrhage within 24 hours having been diagnosed by clinical manifestations and head computed tomography were recruited.All enrolled patients had no history of heart diseases and no complications such as cardiac insufficiency and arrhythmia.Twenty sex and age matched patients with essential hypertension were selected as controls.All patients and controls underwent echocardiographic investigation and at the same time blood was drawn for the quantitative determination of plasma BNP concentration.Results 1) The mean of LVEF in cerebral hemorrhage group was much lower than those in the hypertension group(41.33±15.03)% vs(59.20±15.03)%,P<0.01;2) The correlation coefficient between volume of cerebral hemorrhage and plasma BNP concentration in cerebral hemorrhage group was 0.13(P>0.05).There was no correlation between volume of cerebral hemorrhage and all cardiac functional parameters in cerebral hemorrhage group(P>0.05).There was no correlation between plasma BNP concentration and cardiac structural and functional parameters in cerebral hemorrhage group(P>0.05).Conclusion Acute cerebral hemorrhage can cause left ventricular systolic dysfunction and can cause left ventricular diastolic dysfunction as well.There is no correlation between the volume of cerebral hemorrhage and all cardiac functional parameters.There is no correlation between the BNP concentration and changes in cardiac structural and functional parameters.

    Objective To evaluate the feasibility and clinical outcome of endoscopic axillary lymph node dissection with liposuction(E-ALND) in 56 patients with breast cancer.Methods From October 2005 to February 2007,A total of 56 patients with breast cancer were treated with E-ALND.The clinical data of the patients were analyzed retrospectively and compared with those of 37 patients who had undergone traditional axillary lymph node dissection(T-ALND)during the same period.Results For patients with breast cancer stageⅠand Ⅱ E-ALND(n=56) and T-ALND(n=37) were performed.No case in E-ALND group had been converted to T-ALND,and no incidental injury and subcutaneous emphysema or fat emboli were encountered.There was no significant difference between the two groups in term of the number of resected lymph nodes,the total volume and duration of drainage.However,the blood loss,length of incision and operating time in E-ALND group were significantly reduced as compared with those in T-ALND group(128.1±24.0 mL vs 194.8±59.1 mL,P<0.01;12.9±1.3 cm vs 18.9±1.8 cm,P<0.01 and 96.3±24.3 min vs 80.6±14.4 min,P<0.01 respectively).Median follow-up for the 56 patients was 15.7 months ranging from 12 to 27 months.No wound infection was observed.Two patients had developed numbness and pain in the upper arm and 1 restriction of the shoulder motion in the E-ALND group,which were much lower than those in the T-ALND group(3.6% vs 24% and 1.8 vs 16.2%,P<0.05 respectively).No axillary recurrence and port-site metastases were noticed.Conclusion E-ALND technique is feasible and safe in selected patients with breast cancer,compared with the T-ALND.Furthermore,it requires a smaller incision and has better cosmetic effect and minor morbidity.However,the impact of the procedure on oncology needs to be further verified.

    Objective To assess the pectoralis major muscle function after modified radical mastectomy by muscle strength,ultrasonogram and electromyogram,and also with an evaluation of the significance of preservation of the anterior thoracic nerve.Methods Fourty-six patients with breast cancer who had undergone modified radical mastectomy from Jan 2001 to Dec 2003 were analyzed retrospectively.The patients were divided into four groups:incomplete preservation of the anterior thoracic nerve+Auchincloss's operation group(n=13),incomplete preservation of the anterior thoracic nerve+Patey's operation group(n=7),complete preservation of the anterior thoracic nerve+Auchincloss's operation group(n=20),complete preservation of the anterior thoracic nerve+Patey's operation group(n=6) group.Incomplete preservation of the anterior thoracic nerve was defined as the medial pectoral nerve or lateral pectoral nerve being partially injured.The function of the pectoralis major muscle was assessed by examination of the ipsilatera upper limb muscle strength,ultrasonogram and electromyogram 3 months or longer after surgery.The results were analysed with the aid of SPSS 10.0 statistics software,P<0.05 is considered as statistically significant.Results There was no difference in the strength of pectoralis major muscle among different groups by the muscle strength evaluation.Ultrasonogram and electromyogram examination however,showed signifcant differences statistically in favour of the groups with the anterior thonacic nerve completely preserved.Conclusion The measurement of muscle strength is unable to accurately evaluate the function of pectoralis major muscle after modified radical mastectomy.However,ultrasonogram and electromyogram are able to examine the function of the pectoralis major muscle much better.Furthermore,the results also indicate that it is very important to completely preserve the anterior thoracic nerve in modified radical mastectomy.

    Objective To investigate the microsurgical treatment of aneurysms of the posterior inferior cerebellar artery(PICA).Methods The clinical data,including manifestations,treatment,operative findings,and prognoses of 19 patients with aneurysm of the PICA who underwent microsurgery 1997.5~2005.5 in Beijing Tiantan Hospital were reviewed retrospectively.Results All the 19 patients received microscopic neurosurgery,5 cases were male,and 14 cases were female,aged from 20 to 59 years,average 36.1±15.7 years.Aneurysms in eleven cases located at the left side,and 8 cases at the right side.There were 15 cases presenting firstly with hemorrhage(manifesting with sudden headache,vomiting,or conscious disturbance),5 cases with SAH(subarachnoid hemorrhage),3 cases with ICbH(intracerebellar hemorrhage),3 cases with IVH(intraventricular hemorrhage),4 cases with SAH and IVH.Four cases had spaceoccupying masses as the initial sign presenting with headache,dizziness,or walking unsteadiness.Diagnoses of all of the 19 cases were confirmed by DSA examination,which revealed saccular aneurysm in 17 cases,fusiform aneurysm in 2 cases.The diameters of aneurysms were from 5 to 20 millimeters.Five cases had their aneurysms located at the first section,1 at the third section,2 at the fourth section,11 at the fifth section.There were 5 cases complicated with other angiodysplasia,3 cases with other aneurysms:1 case with arteriovenous malformation(AVM) at the vermis cerebellum of the same side;1 case with the arteriovenous fistula(AVF) of anterior inferior cerebellar artery at the same side.Posterior-median approaches was performed in 15 cases,and far-lateral approaches in 4 cases.Aneurysmal clipping were performed in 8 cases,aneurysmal clipping and resection in 9 cases,aneurysmal clipping and AVM resection in 1 case,aneurysmal resection after clipping and AVF resection in 1 case.Two patients had minor complications after operation,1 case with slight hydrocephalus,1 case with horizontal nystagmus,other cases were all uneventful.Conclusion The incidence of PICA aneurysm is low,and always presenting with hemorrhage or mass effect in contrast to those elsewhere.If the aneurysm being located at the 1~3 section,the far-lateral craniotomy is performed;and if located in the 4~5 section,posterior-median approach is prefered.The option of operation depends on the shape and the stalk width of the aneurysm,fusiform or wide-neck aneurysm received aneurysmal clipping and resection,while,saccular or tight-neck aneurysm received aneurysmal clipping,if complicated with other angiodysplasia,it is better to managed at the same time.The outcome of microsurgical treatment of PICA aneurysm is good.

    Open-door laminoplasty for spondylotic myelopathy has been documented for over 30 years.This method can decompress the vertebral canal and improve the nerve function satisfactorily through the clinical studies.However,radicular and axial symptoms including neck malaise and stiffness and other problems occur frequently after close-door operation.We used a modified open-door laminoplasty to treat the spondylotic myelopathy,and had achieved satisfactory results.Objective Our purpose is to assess the effects of the modified open-door laminoplasty for spondylotic myelopathy.Methods From October 2003 to January 2006,78 patients had been treated with open-door laminoplasty and 43 cases had been followed-up.The operation were carried out in prone position and under general anesthesia.Titan lag screw had been wrung in the lateral mass of C3,C5,C6 and C7 after the door was opened,No.10 silk nesis had been used to sew the Titan lag screw and the spinous process.The vertebral plate was hang with lateral mass screw to keep the door opening.The end of extensor was repaired by PDS-Ⅱadsorbable suture on C2 processus spinosus.Pre-and post-operative JOA scores had been used to evaluate the clinical results.The axial symptoms and radicular symptoms were observed to evaluate the results,using the SPSS 11.5 software to process the data.Results In the 43 cases of cervical spondylotic myelopathy,mean follow up was 22 months(12 to 36 months).The average pre-OP JOA score was 7.2±3.2 and post-OP JOA score was 14.1±2.1;the improvement rate was 73.1±16.7 percent.Sagital diamete of vertebral canal of pre-operation was(11.8±2.2)mm,the average open-door extent was(16.7±4.9)mm,the range of motion of pre-operation was 44.7±5.1 degree,the range of motion of post-operation was 34.2±4.4 degree,the total range of motion had reduced 17 percent after operation(the highest was up to 71 percent according to the literature),the incidence of axial symptoms was 17.83 percent and the incidence of radicular symptoms was 7 percent.According to Odom's scoring system.Twenty-four patients had excellent result,19 good,3 fair.Conclusion The modified open-door laminoplasty for spondylotic myelopathy can enlarge the average open-door extent,increase the range of motion,reduce the radicular and axial symptoms and neck malaise when compared with the traditional method.The modified open-door laminoplasty has achived satisfactory result.However,it problems still remain.Some patients feel neck stiffness after operation in those.Some symptoms have not disappeared completely,especially patients with prolonged course,elder age and disability of bladder or lower limb function.

    Objective To investigate the potential of urinary MMP-2,MMP-9 and MMP-9/NGAL complex as biomakers for screening patients with breast cancer in high-risk female population.Methods Urinary MMP-2,MMP-9 and MMP-9/NGAL complex were detected using substrate zymography gel electrophore and their molecular identity.Urine samples were collected from healthy women(n=60),patients with benign breast disease(n=41) and patients with breast cancer(n=127).Results The expressions of MMP-2,MMP-9 and MMP-9/NGAL complex in urine were detected and expressed as percentages they were 23.33%,15% and 13.33% in normal women;43.93%,39.02% and 36.59% in patients with benign breast disease;64.56%,76.37% and 70.08% in breast cancer patients respectively.Breast cancer patients as compared with normal women(P<0.001),or patients with benign breast disease(P<0.01) showed significant higher levels.Conclusion Detection of all three urinary MMP-2,MMP-9,and MMP-9/NGAL complex may be useful in early diagnosis of breast cancer in high-risk patient population.

    Objective To explore the diagnosis,treatment and prognosis of the optic nerve glioma.Methods Sixteen cases with optic nerve glioma from 2003 to 2007 were collected and their clinical manifestations were summarized and analyzed.Results There were 9 females and 7 males;aged 2~53;8 right side,4 left side,4 bilateral;decline of visual acuity noted in 15;defects of visual field in 12;changes in ocular fundus in 7;headache in 4;ophthalmalgia in 2;exophthalmus in 1;intracranial hypertension in 2;sexual precocity in 1.All cases had positive results in CT and MRI.All cases had undergone operation.Three had postoperative irradiation.Eight cases resulted in blindness,death in 1 and decline of visual acuity in 7.Pathologic diagnosis:astrocytoma of optic nerve in 14,glioblastoma of optic nerve in 1,without pathologic diagnosis in 1.Conclusion Optic nerve glioma is often seen in children under 10,it is usually benign and self-limited.When occurs in adults,it is often malignant.It can cause varied symptoms and signs in eyes and intracranial complications.Comprehensive therapeutic methods should be adopted,including operation,radiotherapy and chemotherapy,etc.Prognosis is dependent on the range involved and the nature of the tumor.

    Objective To investigate the efficacy of the modified xuanfudaizhetang decoation in the treatment of gall reflux gastritis.Methods Sixty-one gall reflux gastritis patients were divided into treatment group and control group randomly,The treatment group took the modified xuanfudaizhetang decoction,and the control group took western medicine such as omeprazole,prepulsid,sucralfate.After treatment,the patients' clinical symptoms,changes in bile reflux under gastroscoply and gastric histology were examined.Results The cure rate and the total effective rate were 73.5% and 97.1% in the treatment group,compared to 52% and 78.7% in the control group.Comparison between these two groups showed significant difference(P<0.05).The improvement of the symptoms in treatment group was greater compared to the control group.The restoration of the histological changes was more pronounced in the treatment group than in the control group.Conclusion Modified xuanfudaizhetang decoction showed better therapeutic effect in treating gall reflux gastritis.
