    A Study on the Effect Factors of the Primary Culture of Laryngeal Squamous Cell Carcinoma

    Objective To study the factors which affect the growth rate of the primary culture of the laryngeal squmous cell carcinoma. Methods Seventeen specimens of resected laryngeal squamous cell carcinoma tissues were obtained for primary cell culture and each of them was dissected in three pieces. Three pieces were dissociated by shearing, collagenase and physical methods, respectively. The results of these three methods and other factors were analyzed by χ2 test. Results Cell growth rate were 64.7% in shearing group, 58.8% in collagenase group and 11.8% in physical group. The growth rate between the shearing group and collagenase group had no significant difference (P=1.000) while that of the physical group was much less than the other two groups (P=0.004, P=0.008). The tumor differentiation affected the growth rate. For well differentiated tumors the growth were 33.3% and for mildly/poorly differentiated ones the rate were 90.9%(P=0.028). In the study, there was no correlation between the TNM stage and the growth rate. Conclusion The growth rate is higher with tumors from patients with poor differentiated cells. The shearing method has high cell growth rate and less procedure, so it is the best method.

    Objective To evaluate the efficacy and safety of the standardized dust mites vaccine for the treatment of allergic rhinitis. Methods Thirty patients with allergic rhinitis caused by house-dust mites (HDM) were selected in this self-contrast study. Clinical efficacy was evaluated by symptom and sign score after six months of specific immunotherapy and compared with pre-treatment scores. Results Twenty eight patients completed the study. A clinically significant reduction in symptom and sign score in these 28 patients comparing with that of pre-treatment was founded, and the difference was significant(P<0.01). 92 local adverse reactions and 8 systemic reactions had occurred in total 566 injections, and no fatal systemic reaction occurred. Conclusion Standardized allergen specific immunotherapy is an effective treatment in patients suffering from allergic rhinitis due to house-dust mites.

    Objective Polymerase chain reaction (PCR) of internal transcribed spacer region (ITS) is an exquisitely sensitive assay that can detect the DNA of fungal elements. The aim of this study was to compare the sensitivity of conventional culture and techniques using PCR. Methods The samples of secretion were collected from 18 patients diagnosed as fungal sinusitis clinically and 20 cases of control group with chronic sinusitis. Fungal-specific PCR analysis and standard culture were performed on every sample. χ2 analysis was used to test for statistical differences between the two groups. Results ITSPCR analysis detected fungal DNA in fourteen(78%) patients with chronic fungal sinusitis and eight cases in control group while the positive results of standard cultures were 44%, respectively. Conclusion ITS PCR is more rapid and more sensitive technique than nasal swab culture in detecting the fungi in sinus secretion. The study suggested that it is an effective method in diagnosis of fungal sinusitis.

    Objective To evaluate the features of auricle reconstruction by Medpor framework plantation, and to explore the reasons for framework exposure. Methods 31 cases of microtia, who had been treated by using the Medpor framework in Beijing Tongren Hospital during April 2000 to October 2002, were studied retrospectively. Results 31 cases were cured without any complications after one stage and all discharged after two weeks of the operation. The follow-up result showed that 11 cases had the framework exposed partially, among which 8 cases were treated locally, the other 3 cases' frameworks taken out. The auricle shape of 27 cases had good cosmetic result. Conclusion The main reason of framework exposure includ the skin tension?thin skin flap or the effect of external force. In spite of the high rate of framework exposure, Medpor framework is still a good substitute for the rib cartilage framework.

    Objective To investigate the patterns of Reinke's edema of vocal fold and its outcomes following CO2 laser Phonomicrosurgery. Methods 65 patients with Reinke's edema were evaluated by clinical manifestation, acoustic analysis, aerodynamic analysis and videostroboscopic examination for preoperation and post operation. Results The Reinke's edema were mainly senior patients. The vocal fold vibrate with increased mucosal wave and hypertrophy of false fold presented to compensate incomplete glottal closure more frequently in Reinke's edema. There was significantly difference in acoustic analysis between normal and lesions(P<0.01). F0 of Reinke's edema was lower than that of control group(P<0.05). Vocal function recovered three months after operation steadily. Conclusion Reinke's edema is located within the whole cover layer of the vocal fold. CO2 laser phonomicrosurgery can control the range of the operation delicately. Vocal function can recover steadily three months after operation.

    Objective To investigate the expression and the role of costimulatory molecules B7:CD28/CTLA-4 in erythema nodusum of patients with Behcet disease. Methods Erythema nodusum from 5 patients with progressing Behcet disease and normal skin from 2 healthy individuals were analyzed by immunohistochemical examination for expression of CD4, CD8, CD19, CD68, HLA-DR, B7-1, B7-2, CD28 and CTLA-4 molecules. Labelled cells were visualised using either single or double staining techniques. Results Approximately 90% of epithelial cells in erythema nodusum expressed B7-1 and B7-2 molecules, but no expression of CD28 and CTLA-4. CD4+Tcells, CD8+T cells, CD19+cells, CD68+cells, HLA-DR+cells, B7-1+cells, B7-2+cells, CD28+cells and CTLA-4+cells were increased significantly in the dermis and subcutaneous tissue. Double staining showed that CD28 molecules were expressed on 100% of CD4+T cells, CTLA-4 molecules were expressed on 47.9% of CD4+T cells, respectively. All CD68+ cells expressed B7-1 and B7-2 molecules. B7-1 and B7-2 molecules were expressed on 74.5% and 60.8% of HLA-DR+cells, respectively. Conclusion CD28 and CTLA-4 molecules are mainly expressed on CD4~+T cells, B7-1 and B7-2 are mainly expressed on macrophages, antigen-presenting cells and keratinocytes. B7:CD28/CTLA-4 costimulatory pathway can induce the function and activation of macrophages and T lymphocytes, and play an important role for the incidence, diffusion and consistence of inflammation in erythema nodusum.

    Objective To study clinical effect of phacoemulsification and goniosychialysis for angle-closure glaucoma with cataract. Methods 226 eyes of 161 patients were undergone phacoemulsification and IOL implantation, combined with goniosychialysis. The main outcome measures were pre-operation and post-operation change of anterior chamber angle, depth of anterior chamber, visual field and intraocular pressure. 186 eyes were followed up within 3 years, 40 eyes over 3 years. Results The intraocular pressure of 1 eye raised again. The management of trabeculectomy could not control the IOP, after silicon tuber implantation the pressure was controlled. The anterior chamber angles of all patients were open, the depth of central anterior chamber deepened average 1.1 mm, the depth of peripheral anterior chamber average 0.2 mm. The visual fields of 215 eyes had no depression except for 1 eye with raised intraocular pressure. Conclusion Phacoemulsification combined with goniosychialysis is an effective surgery for angle-closure glaucoma except for inflammative glaucoma, the concrete indication of the surgery should be explored further.

    Objective To observe the histopathological manifestations and the evolvement processes of different retinal reactions caused by transpupillary thermotherapy (TTT). Methods Transpupillary thermotherapy (TTT) was performed to the retina of normal rhesus monkey. The histopathologic changes of different laser spots no visible reaction, mild reaction and intense reaction at different time 1 d, 1 week, 2 week, and 1 month were observed. Results No change was found in no visible reaction laser spots by light microscope. The histopathologic changes of mild reaction were confined to the outer retina. The histopathologic changes of intense reaction involved the whole layer of retina. The histopathologic changes turned to stable after 2 weeks. Conclusion Different TTT reaction can result in the different extent damage to retina. Suitable reaction should be performed according to the nature of the diseases.

    Objective To observe the inhibition of neovascularization by jiangtangmingmu capsule on mouse diabetic retinopathy. Methods Male C57BL/B mice was injected with STZ to induce the diabetic mouse model, which was accepted to be nice model to study the diabetic retinopathy. Three groups were treated with serious doses of jiangtangmingmu capsule, the doses were 18, 9, 4.5 folds of clinical application respectively. Two positive treatment control groups were subjected with two other clinic approved medicine. Medicines were applied intragastric feeding daily. Serum glucose and body weight were observed periodically. After 80 d treatment, retina sections were stained by HE method and neovascular were counted under biological microscope. Results Diabetic mice induced by STZ were observed as raised serum glucose, and body weight lost significantly. Retina sections with HE stained were counted as increased neovascular numbers. All treated groups, including the serious doses of jiangtangmingmu capsule, two approved medicines, were observed as serum glucose decreasing and body weight increasing compared to the untreated diabetic mice. And the neovascular counting was decreased after the medicine treatment. Conclusion Jiangtangmingmu capsule can inhibit retinal neovascularization of diabetic mice induced by STZ.

    Objective To evaluate the effects of calmodulin antagonist W-7 on the proliferation of in vitro cultured human Tenon's capsule fibroblast cells for finding new antiproliferative drugs applied for the glaucoma filtration surgery. Methods Human Tenon's capsule fibroblast cells were cultured in DMEM medium containing 10% fetal bovine serum at 37 ℃,5% CO2. For growth inhibition assays, cells lie in logarithm growth period were seeded into 96 well plate, and treated with W-7 and MMC, the inhibitory effect was determined by MTT(tetrazolium bromide) method. The mitochondrial metabolic ability of the drugs treated cells was also examined by MTT assays. Results W-7 caused significant inhibition effect of fibroblast proliferation in dose and time dependent manner, which were perhaps the causes of cell apoptosis in human Tenon's capsule fibroblast cells. The MTT assay showed that W-7 treated cells had mitochondrial metabolic ability while MMC treated cells lost this ability. While the cell membrane integrity of W-7 treated cells was superior to MMC treated cells, and it may cause slight inflammatory reaction. Conclusion There is a good prospect of W-7 for clinical application.

    Objective To investigate the associated factors with the occurrence of subsensory choroidal neovascularization (CNV). Methods The images of optical coherence tomography (OCT) of 95 cases (105 eyes) with CNV diagnosised by fundus examination, flurescein angiography (FA) were analysed. Results The percentage of subsensory retina CNV was significantly higher in the exudative age-related macular degeneration (AMD) cases with course within 2 months than course over 2 months( χ2=6.687,P=0.010). The percentage of subsensory retina CNV was significantly higher in the non-exudative AMD cases than that of the exudative AMD cases (χ2=42.992,P=0.000). Conclusion Subsensory retina CNV mainly occur in non-exudative AMD cases and AMD cases with short course.

    Objective To evaluate the effect of ocular micro-endoscope to treat complicated ocular trauma. Methods 92 patients with severe ocular trauma were treated with complicated post-segment operations under ocular micro-endoscope, including lens-vitrectomy, cyclophotocoagulation, intraocular foreign body extraction, retina reattachment, peripheral retina examination. Results 52 patients with post-segment disorders and severe cornea cloudy were undergone with vitrectomy under the micro-endoscope. All intraocular foreign bodies were extracted. Those patients with retina detachment were tamponated with silicone oil and their retina got anatomic reattachment. 40 patients with secondary glaucoma were treated with cyclophotocoagulation. Theintraocular pressure of 34/40 patients were under control and the mean intraocular pressure was ( 16.79±3.01) mmHg [(2.24±0.40)kPa].The intraocular pressure in 6/40 patients were out of control and they were retreated with trans-sclera photocoagulation. Conclusion Operations in ducted by micro-endoscope are useful for the patients,who got post-segment disorders complicated with cornea-opacity or refractory glaucoma.

    Objective The study focus on the utilization of hard-palate and a pedicled flap above the eyebrow for treating completed eyelid defect. Methods The objects were eighteen patients with 18 sick eyes. After the different layers of eyelid were completed separated, hard-palate and the pedicled flap were harvested. Both of them were used to reconstruct the eyelid. Results Following up over 18 months, none of the 18 eyes were failed. Histologic observation of grafted hard-palate mucous revealed 6 to 7 layers of squamosal epithelium which also had similar tightened subepithelial collagen fibrils of tarsal plate. Conclusion The combines transplantation of hard-palate and this flap can supply a reconstruction of palpebral skin and tarsal plate on patients with completed eyelid defect by only one step.

    Objective To analyze the treatment timing and efficacy of acute retinal necrosis (ARN) syndrome. Methods 35 patients (52 eyes) diagnosed with ARN syndrome undergoing systemic medication, retinal photocoagulation and vitreous surgery were reviewed retrospectively to evaluate the timing and efficacy of the treatment. Results In the patients who took medicine atthe first 2 weeks, 17 of 22 eyes (77%) avoided retinal detachment and 14 eyes had a final visual acuity better than 0.3. Nevertheless, only 12 of 31 eyes (39%) with medicine and retinal photocoagulation were spared from retinal detachment, and 4 eyes underwent vitrectomy for sever proliferative vitreoretinopathy. Retinal reattachment was achieved in 20 of the 24 eyes with vitrectomy. 18 eyes of those with retinal detachment before surgery had visual acuity from light perception to 0.05, others from 0.05 to 0.2. Conclusion It is suggested that prompt treatments, such as medication, retinal photocoagulation and vitrectomy are important managements to ARN syndrome. The visual outcome suggests that the eyes without retinal detachment have better effect. Vitrectomy should be considered before retinal detachment.
    Effects of CoCl2 Pretreatment on Tolerance of Mice Preconditioned to Hypoxia

    Objective To observe the effects of CoCl2 treatment on tolerance of mice to hypoxia. Methods Balb/c mice were injected with CoCl2 3 h before exposure to hypoxia. The animals' tolerance time of each hypoxia exposure and the disappearance time of wave PS of hippocampal slice were recorded. Results The hypoxic tolerance time of CoCl2 pretreated mice was increased but unchanged during the repetitive exposure to hypoxia, whereas the tolerance time of control group was increased as the exposure to hypoxia increased. The disappearance time of wave PS of hippocampal slice in CoCl2 administrated group was longer than that of control but shorter than hypoxic preconditioning group. Conclusion Different molecular mechanisms are thought to be involved in the induction of tolerance to hypoxia between hypoxic preconditioned and CoCl2 treated mice.

    Objective To obsere influence of RGDS peptide on rat platelet aggregation rate and the ERK1/2 phosphorylation induced by different agonists in vivo and study the relationship between the change of ERK1/2 phosphorylation and the change of platelets aggregation with RGDS peptide. Methods Maximal aggregation rate(MAR) was measured with nephelometry; ERK1/2 phosphorylation was detected by Western blot. Results The MAR and phosphorylation of ERK1/2 were inhibited by RGDS peptide. Conclusion The phosphorylation of ERK1/2 was one of the cellular signal transduction mechanisms of the platelets aggregation; and the inhibition of ERK1/2 phosphorylation of may be one of the mechanisms that RGDS peptide antagonize thrombus.

    Objective To establish a laboratory method for rapid, simple and sensitive diagnosis of SARS. Methods Culture supernote of 34 viruses strains and sera of SARS patients were assayed by CLIA. Results The experiments indicated high specificity and sensitivity to viruses strains, reactivity with 6 SARS strains and not cross-reactivity with 11 others strains, the detectability was on more then 6.3×101 PFU/mL of SARS-CoV strain. For the 63 SARS patients, positive rate was found to be 100% during the disease day of 6~10 and for the 1 066 normal sample, negative rate was 99.35%. Conclusion The sensitivity and specificity of CLIA for SARS-CoV detection is higher satisfying. It is rapid and simple. It might be used to earliest diagnosis of SARS-CoV infection.

    Objective To amplify the gene sequence of CD14, analyze the structure and bio-information of the cloned genes. Methods Human soluble CD14(sCD14) gene cDNA fragment was amplified from total RNA extracted from U937 cells by RT-PCR. The bio-information of cloned CD14 gene and translated protein was then analyzed, DNA and amino sequence, protein prosite and stabilization, alpha-helix/Deleage and Roux were identified. Results The amplified nucleotide sequence fragment consistent completely with the published human CD14 sequence. Moreover, we knew clearly gene structure and bio-information parameter of human soluble CD14 according the sequence. Conclusion The co-index of the gene of CD14 is achieved successfully. These results make it possible to identify Human recombinant CD14 gene.

    Objective To investigate myocardial damage, the alterations of plasma, and myocardial angiotensin Ò and myocardial AT1 receptor as well as the effects of bisoprolol after brain injury in rats. Methods Brain injury was induced by weight-drop technique and pretreated with bisoprolol. Plasma angiotensinÒ level was determined using RIA. Myocardial angiotensinÒand AT1 receptor were detected by immunohistochemical method. Myocardium-type creatine kinase isoenzyme (CK-MB) was measured. The morphologic changes of myocardium were observed with light and electronic microscope. Results Plasma angiotensin Ò and serum CK-MB increased at 24 h after brain injury. AngiotensinÒ and AT1 receptor in myocardium also increased. Degeneration and necrosis of myocardium were detected with light and electronic microscope, which included increasing and swelling of mitochondria and disruption of cardiac muscle fiber. Plasma angiotensin Ò and serum CK-MB decreased in bisoprolol group compared with brain injury group. Relief of myocardial damage was also observed with light and electronic microscope in bisoprolol group. Conclusion Brain injury can lead to myocardial damage. RAS maybe plays an important role in the mechanism of myocardial damage after brain injury. Bisoprolol can prevent myocardial damage after brain injury.

    Treatment of Postoperative Hyponatremia in Children Associated with Sella Region Tumor

    Objective To investigate the etiology, regularity and treatment of postoperative hyponatremia of sella region tumor in children. Methods Clinical studies of 81 patients with postoperative hyponatremia of sella region tumor were reviewed retrospectively. Recording serum sodium on the operative day and postoperation daily, corresponding therapy according to the serum sodium and patient's condition was also recorded. Results Purely hyponatremia was observed in 81 of 450 cases. Mild moderate and severe cases hyponatremia were 28(34.6%), 35(43.2%) and 18(22.2%), respectively. 26 cases(32.1%)had disturbance of consciousness. 50.6%(41/81) was occurred on operative day and the first day after operation. 83.9%(68/81) was recovered within 8 d. Conclusion SIADH and CSWS are cardinal etiological factors of hyponatremia in children associated with sella tumor. Grasping the regularity and etiology can redress the sodium irregularity effectively and shorten the pathological process.

    Objective To compare the clinical results in terms of operative lesions among treatments via different approaches which include frontal transcortical(n=19), frontal intergyral(n=11), and frontal transcallosal(n=11) approaches respectively. Methods The removal extent of brain tissue, the removal degree of tumer and the incidence of postoperative morbility were analyzed with statistical method. Results No significant difference among the removal degree of lesions, but significant difference among the removal extent of brain tissue and the rate of postoperative morbility. The highest postoperative morbility rate occur with the patients operated via frontal transcortical approach (55.6%), but the lowest postoperative morbility rate occurs with the patients via frontal transcallosal approach (9.1%). The postoperative morbility rate of patients via frontal intergyral approach (18.2%) is less than that operated via frontal transcortial approach; but higher than that operated via frontal transcallosal approach. Conclusion Operational treatment of the septum pellucidum via transcallosal approach and intergyral approach accord with the microinvasive concept. It is appropriate to select transcallosal approach in frontal part as the first choice for septum pellucidum in terms of operative lesions.

    Objective To investigate the relation between the polymorphism of endothelial nitric oxide synthase (eNOS) gene 27 bp variable number of tandem repeats (VNTR) and diastolic blood pressure in hypertensive patients and its possible mechanism. Methods Genotypes of 278 hypertensive patients were determined by polymerase chain reaction (PCR) and agarose gel electrophoresis and gene sequencing was performed at the same time; fasting serum nitric oxide metabolite (NOx) were measured by nitrate reductase. Results The levels of NOx in ab+aa genotype were lower than that in bb genotype(P<0.05). Diastolic blood pressures of ab+aa genotype were significantly higher than that of bb genotype(P<0.05). Conclusion It is suggested that the polymorphism of eNOS gene 27 bp VNTR may be involved in the etiology of the elevation of diastolic blood pressure by reducing NO release of endothelium.

    Objective To investigate the effects of atrial fibrillation complicated with hypertension on endothelial function. Methods Eighty-eight mild to moderate essential hypertension patients were enrolled. Thirty-four of them were complicated with atrial fibrillation. And patients were divided into two groups: hypertension and atrial fibrillation with hypertension. Plasma endothelin(ET), nitric oxide(NO) and indices of heart structure and function were measured by two-dimensional echocardiography. Thirty-four healthy persons matched in age and gender served as control. Results Left atrial diameter was significantly larger in atrial fibrillation with hypertension than in hypertension. Plasma ET level and ET/NO ratio were higher in atrial fibrillation with hypertension than other groups. And the atrial fibrillation with hypertension group had lower NO level as compared with other groups. Conclusion These findings suggest that atrial fibrillation has effects on atrial remodeling, which is based on hypertension effects. Endothelial function is seriously damaged and decreased in atrial fibrillation with hypertension patients.

    Objective To investigate whether or not survival was decreased after incubation with high glucose concentrations in culture media, and to examine the effects of glucose on the expression of intercellular adhesion molecule-1(ICAM-1)、vascular cell adhesion molecule(VCAM-1) in cultured human umbilical vein endothelial cells. Methods Human umbilical vein endothelial cells (ECV304) were incubated in a culture medium with 11.2 mmol/L, 16.8 mmol/L and 33.6 mmol/L glucoseconcentrations for 24 h, 48 h, 72 h respectively. The trypan-blue exclusion test was used to study the influence of D-glucose on cell survial.Theexpression of intercellular adhesion molecule-1(ICAM-1), vascular cell adhesion molecule-1(VCAM-1) were measured by flowcytometry. Results When D-glucose concentrations rose trypan-blue excluded cells were decreased. HUVEC exposed to a high glucose concentration (33.6 mmol/L) showed a 1.22-fold increase and a 1.27-fold increase in cell surface expression of ICAM-1 after 48 h and 72 h exposure compared with those cultured in medium with a low glucose concentration( 5.6 mmol/L). Conclusion High concentration of glucose can arrest the proliferative response and show that even a short-term exposure of endothelial cells (ECs) to high glucose concentration leads to their activation associated with increased expression of adhesion molecules such as ICAM-1 and that this effect may appears more significantly along with time of exposuring high concentration of glucose.

    Objective To evaluate the efficacy and safety of domestic-manufacture gatifloxacin tablet in the treatment of acute bacterial respiratory and urinary tract infections. Methods A randomized and controlled clinical trial was conducted to compare the gatifloxacin and levofloxacin for the treatment of patients with acute bacterial respiratory and urinary tract infections. The dose of both agents was 200 mg twice daily per os. The duration of the therapy was 5 to 14 d in both groups. Results Forty-three patients were enrolled in this study after excluding 1 case of negative bacterial culture, 1 case with incomplete CRF. The patients were divided into gatifloxacin group(n=25) or levofloxacin group (n=26). The overall effective rates and bacterial clearance rates were 100% and 94.4% in gatifloxacin group, 96.2% and 89.5% in levofloxacin group, respectively whereas the adverse drug reaction rates were 0 and 3.8%, respectively. No statistical significances were found in above data. Conclusion Gatifloxacin and levofloxacin were effective and safe for the treatment of acute bacterial respiratory and urinary tract infections.

    Objective To investigate the diagnositic value of fiberoptic bronchoscopy in simple pleural effusion. Methods Observe the relation between the results of fiberoptic bronchoscopy and final diagnosis in patients with simple pleural effusion from 1998 to 2002. Results 22 patients enrolled, 7 patients with malignant effusion, and 2 patients of the 7 with adenocarcinoma of bronchus were diagnosed with fiberoptic bronchoscopy, 14 with tuberculous pleurisy, 1 with empyema. Conclusion Fiberoptic bronchoscopy is a valuable method to identify simple pleural effusion, particularly in bronchial carcinoma.

    Objective To investigate the feasibility of rat bone marrow stromal cells (BMSC) differentiating into endothelial cells in medium with high concentration rhGM-CSF (recombinant human GM-CSF). Methods Bone marrow cells were obtained from rats bones (10 Sprague-Dawley male rats) with an aseptic technique. Afterwards, all obtained cells were divided into two groups: cells in test group were cultured in 1640 medium with high concentration rhGM-CSF (400 μg/L) in order to induce bone marrow stromal cells into endothelial cells; cells in control groups were cultured in medium with 1640 medium only. Cells on the 8th day were collected for observation with electronic microscopy of ultrastructural characteristics. Results Cells of test group demonstrated specific characteristics by transmission electronic microscopy observation, especially W-P bodies (Weibel-Palade bodies) were observed. There were polygon shaped cells with lots of thin pseudopodium and short micro-floss at the cells membrane under scanning electronic microscopic evaluation, which were coincided with endothelia cells characteristics. Conclusion Bone marrow stromal cells are able to differentiate into cells with endothelial-like characters in the medium with high concentration rhGM-CSF.

    Objective To investigate the relationship between anterior/posterior cerebral artery flow velocity and the collateral flow patterns in patients with middle cerebral artery stenosis or occlusion. Methods 49 patients with unilateral MCA stenosis/occlusion identified by DSA were selected. Calculating the ratio of ACA flow velocity in affected side with MCA flow velocity in unaffected side and ratio of bilateral PCA flow velocity, compared the data collected from the healthy controls. Results According to the data, we found the patients'dVACA/nVMCA(1.117±0.298)is significantly higher than the controls(0.801±0.147), P=0.027. Logistic regression analysis showed that the stenosal extent and value of dVACA/nVMCA are two independent predicting factors of whether ACA collatery recruited. Conclusion TCD findings can partly reflect the collateral patterns in patients with MCA stenosis or occlusion.

    Objective To study the effects of severe acute pancreatitis on the level of serum insulin and glucagon in rats. Methods SAP model was produced by retrograde injection of 3.5% sodium taurocholate into the cannulated biliopancreatic duct, and rats were sacrificed at 6, 24, 48, 72 h after induction of pancreatitis. The pancreatic tissue specimens were collected to assess the pathological score of exocrine part in terms of tissue damage and Bcl-2 expression was observed with immunohistochemical examination. Prior to sacrifice, blood samples in 6, 24, 48, 72 h were taken from the inferior vena cava to measure serum insulin and glucagons with radioimmunoassay (RIA). Results Tissue damage got worse with time passing and maximized at 24 h after SAP model established. With the development of disorder, the pancreas presented necrosis, hemorrhage and severe leukocytes infiltration. The serum insulin level markedly decreased at 24, 48, 72 h, and the level of glucagon significantly increased at 6, 24, 48, 72 h compared with the experimental control group and normal control group. The differences were significant statistically. Immunohistochemistry examination showed that the Bcl-2 expressed in islets of SAP rats after 6 h, however disappeared after 24 h. Conclusion Severe acute pancreatitis impairs both the pancreatic exocrine and endocrine functions. On the other hand, the Bcl-2 expression in immunohistological examination indicated that there might be a protective mechanism against the endocrine dysfunction in SAP, in which Bcl-2 was involved.

    Objective To investigate the long-term follow up results of patients with duodenal ulcer perforation managed by simple suture and pharmacal management. Methods All discharged patients (77 patients) with perforated duodenal ulcer consecutively operated on with simple closure underwent follow-up in our hospital from January 1991 to December 2000. According to follow-up data, the long-term results of recurrence of ulcer symptoms for these patients with perforated duodenal ulcer operated on with simple closure were determined. Results The rate of symptomatic ulcer recurrence (VisickⅢ~Ⅳ) and Existing rate of ulcer of postoperative patients with endoscopy examination in four to eight weeks were 15.4%(10/65), 17.5%(7/40) respectively, and reoperation rate was 1.5%(1/65). Conclusion The long-term results indicated that it had a good effect to prevent the recurrence of ulcer symptoms that H2 receptor antagonists and proton pump inhibitors and drugs of anti-helicobacter pylori were used in patients with perforated duodenal ulcer operated on with simple closure.

    Objective The hypothesis was under investigation that verapamil as a Calcium-channel antagonist, is introduced to prevent excretion of anticancer drugs from cells excerting reversing effects on drug resistance in bladder cancer. Methods MTT and HPLC method are applied to detect the alteration on the sensibility of cancer cells to anticancer drugs before and after introducing verapamil. Results Verapamil has no effect on the growth of bladder cancer-sensible cells(EJ). For the drug-resisted bladder cancer cells (EJ-R), effects of anti-cancer drugs are enhanced with the increasing concentration of verapamil. Conclusion Glucoprotein P-GP_ 170 on the membranes of bladder cancer cells can excrete anti-cancer drugs from the cells against concentration gradient. Verapamil blocks Calcium channels thereby reduce excretion of anti-cancer drugs by P-GP170. By guaranteening the concentration of anti-cancer drugs in the cancer cells, drug resistance of cancer cells is reversed.

    Objective To investigate hemodynamics of elderly patients with propofol application before retrobulbar nerve block in department of ophthalmology. Methods There were 40 patients in department of ophthalmology to be operated, age≥65 years, ASA Ⅱ~Ⅲ grade. They were randomly allocated into A and B two groups. A was control group(n=20), B was propofol group(n=20). The patients of A group were routinely operated with retrobulbar nerve block without general anaesthetic. The patients of B group were injected propofol (0.4 mg/kg). Heart rate, blood pressure and SpO2 of patients were in two groups in peri-operational period. Results Heart rate, systolic pressure and diastolic pressure increased significantly observed in patients of A group 1 minute after nerve block(P<0.05). They gradually returned to basic level after 2~5 min. Heart rate, systolic pressure and diastolic pressure did not change much in patients of B group every time points. There are significantly statistical difference between patients of A group and patients of B group in heart rate, systolic pressure and diastolic pressure 1 min after nerve block( P< 0.05). Conclusion Injecting propofol(0.4 mg/kg) into retrobulbar nerve block for elderly patients is advantageous in terms of qick effect, well sedation, stable hemodynamics, and less respiration disturbance.

    Objective To investigate the reasons of failure with mifepristone in ectopic pregnancy treatment, and find theoretical reference on receptor level for anti-progestogen pharmacal therapy in clinical ectopic pregnancy treatment. Methods All the selected cases were divided into three groups, the mifepristone treatment failure group, the operation group and the normal pregnancy group. The decidua reaction in all the groups were observed with HE staining under microscope. The expressions of ER and PR in fallopian tube wall of pregnancy site during tubal pregnancy were measured with immunohistochemical method. The failure cases of mifepristone treatment were also measured. Results The decidua reactions in the mifepristone treatment group and the operation group were less than that in the normal pregnancy(P<0.05). The expression of ER was of no significant difference among the three groups. The expression of PR in the mifepristone treatment failure group was less than in the operation group, while the expression of PR in the normal pregnancy group is more than in other two groups.Both the differences were significant. Conclusion It was noticed of a weak tubal tissue, less decidua reaction, and low expression of PR in tubal pregnancy. So, it was not theoretically reasonable for mifepristone treatment to tubal pregnancy which may have little target tissue of mifepristone.

    Objective To study the associated clinical factors and their impacts on the FGR. Methods Retrospective analyses of clinical data were performed in 134 cases of pregnant woman with FGR. Results The clinical factors of FGR include pregnancy-induced severe hypertension syndrome, heart and lung diseases, serious placenta and cord abnormality, twin pregnancy, oligohydramnios etc. In the FGR group the occurrence of above-mentioned factors are obviously higher than control group(P< 0.01). Rural residents or those without antenatal care are also significantly( P< 0.01) higher than control group. In the FGR group the incidence of following diseases were also obviously higher( P< 0.01): including new born suffocations, still-born baby, still birth and new born disease. Conclusion Pregnancy-induced hypertension syndrome, heart and lung diseases, blood diseases, serious placenta and cord abnormality, twin pregnancy, oligohydramnios are the primary clinical factors leading to FGR. Living environment and prenatal care are also important factors related to FGR. FGR is one of the important reason leading to still-born baby or new born baby disease or death.

    Objective To study the different incidence of dry socket affer the third mandibular molars extraction using two different methods: implantology surgical unit and conventional method. Methods 156 patients were selected randomly, 156 impacted lower third molars on one side were removed with implantology surgical unit method; 107 lower 3rd molars on the other side by conventional chisel method (self control). The number of dry sockets were recorded. Results The incidence of dry socket in implantology surgical unit group was significantly lower than the conventional group. There were significant differenes between incidence of dry socket of the two methods (P=0.013). Conclusion The implantology surgical unit method is safer, more time-saving and less traumatic than the conventional chisel bone method. The study indicated the former is effective in dry socket prevention.

    Objective To investigate the epidemic distribution and drug resistance of the clinically-isolated bacteria in intensive care unit(ICU)of Beijing Children's Hospital. Methods All the bacterial isolations from 1999 to 2003 were analyzed. Tests were performed according to the guidelines of NCCLS. Results Gram-negative bacteria became common in pediatric patients of ICU. The main Gram-negative bacteria included Pseudomonas aeruginosa; Acinetobavter spp. and Klebsiella spp. Gram-positive were became important pathogens of nosocomial infection. The main pathogens were Coagulase-negative Staphylococci(CNS)(50.8%) and Enterococcus(12.4%) in blood, and Pseudomonas aeruginosa(26.0%), Acinetobavter spp. (19.8%), Klebsiella pneumoniae(19.8%) in respiratory tract,respectively. The Methicillin-resistant coagulase-negative Staphylococci (MRCNS) rate were 76.6% and Methicillin-resistant Staphylococcus aureus(MRSA) rate were 1.9%. Extended-spetrum β-lactamase stains counted for 54.0% of E.coli and 80.7% of K.pneumoniae from 1999 to 2003 in ICU. Conclusion Imipenem remains highly active against Gram-negative bacteria. Bacteria isolated from ICU exhibit a high resistance to penicillins and cephalosporins. Multi-drug resistance of bacteria is a serious problem in ICU. It is very important to monitor the distribution of bacteria and their resistance to antibiotics in pediatric patients of ICU.

    The recent research about the model of activating signaling pathways in the natural killer (NK) cells allows us to shed new light on how NK cells function. Binding of activating receptors to its ligand(s) will lead to activation signals to proceed, resulting in activation of SLP-76, WASP and PI3K. Activated SLP-76 will recruit and bind many downstream molecules including PLC-γ, Itk and Vav, whereas activated WASP will recruit and bind Grb-2, Vav, WIP, Arp2/3 and Cdc42-GTP, among others. Nck, which binds to both SLP-76 and WASP, will transport the multimolecular signaling complex to cytomembrane where activated Arp2/3 and PLC-γ will partake in actin polymerization, cytoskeletal remodelling, NFAT gene transcription and cytokine secretion. Activation of PI3K will lead to granule polarization through Rac/PAK/MEK/ERK pathway.
