    Objective To study the role of the cyclooxygenase (COX)2 in the genesis of squamous cell by means of observation on its expression in oral leukoplakia, lichen planus and squamous cell carcinoma.Methods COX-2 expression was observed in 51 cases of OLK (15 OLK, 36 OLK with dysplasia), 34 cases of OLP ( 19 OLP, 15 OLP with dysplasia),52 cases of SCC and 10 cases of normal oral mucosa by immunohistochemistry. The number of positive cells was counted by image analysis system and COX-2 expression was analyzed by statistical method.Results The strong expression of COX-2 in the OLK was 6.7% (1/15); 50% (18/36) in the OLK with dysplasia; 15.8% (3/19) in the OLP; 33.3% (5/15) in the OLP with dysplasia; and 7.7% (4/52) in the SCC respectively. The positive rate was higher in the OLK with dysplasia than in the OLK or SCC (P<0.01). The higher positive rate was detected in the OLP with dysplasia than in the OLP or SCC (P<0.05). There were no significant differences between OLK, OLP and SCC (P>0.05).Conclusion COX-2 overexpression occurred in the early stage of oral carcinogenesis, it could be an important target for the chemical prevention and therapy.

    Objective By comparing the histopathological characteristics of gingiva taken from untreated smoking and non-smoking patients with advanced periodontitis, to investigate the possible role of smoking in the genesis and development of periodontitis.Methods The gingival biopsies of untreated smoking and non-smoking advanced patients with periodontitis were performed, and routine histopathological and immunohistochemical methods were used to assess the general histological characteristics and the changes of gingival blood vessel density and circumference respectively.Results Compared with non-smokers, the hyperplasia of pocket epithelium in smokers was continuously extending toward root apex, and the layer of keratin is thicker. The difference between the vascular density of smokers and non-smokers was not significant, however, smokers showed a higher proportion of small and a lower proportion of large blood vessels.Conclusion These results indicated that smoking maybe deepen the pocket and disturb the reattachment of periodontal tissue by prompting the hyperplasia and keratinization of pocket epithelium. The incremental proportion of small vessels proportion maybe result in a decline of the total blood flow in gingiva, which may in turn impair the resistibility and restoration of tissue.

    Objective To investigate the expression of EDA region of fibronectin(FN) in salivary adenoid cystic carcinoma(SACC) cells and deduce the relationship between EDA and SACC biological characteristic.Methods Immunohistochemistry and reverse transcriptase polymerase chain reaction(RT-PCR) techniques were used to detect EDA region proteins and mRNAs in different metastatic potential SACC cells.Results The expression of EDA region proteins and mRNAs in high metastatic SACC-LM cells were higher than those in low metastatic SACC-83 cells.Conclusion The expression of EDA region of FN may play an important role in the invasion and metastasis of SACC.

    Objective To study in vitro the anticaries effect of ethanoic extracts of propolis varnish on prevention of artificial caries in primary teeth.Methods 48 extracted primary teeth were randomly divided into 4 groups, dealed with 10 g/L propolis varnish, 100 g/L varnish, fluor protector respectively together with a control group. After 96 h of incubation with S. mutans and S. sobrinus, the caries lesion depth and mineral losses of each group were analyzed.Results Both propolis varnish of all concentrations and Fluor Protector can significantly prevent the formation of artificial caries on primary teeth compared with negative control (P<0.001); while there was no significant difference among propolis varnishes and the fluoride varnish in protecting primary teeth from cariogenic challenge (P>0.05).Conclusion Propolis varnish can prevent the development of artificial caries of primary teeth, and this kind of propolis varnish has a similar anticaries effect with fluoride varnish.

    Objective To evaluate the long term pulpal pain response in terms of time-sequenced changes to the stimulation of composite filling.Methods Immunohistochemistry method was used to stain the substance P(SP) in pulpal tissue which could directly reflect the activation of nerve by stimuli.Results In the area of odontoblast and their protrusions, the SP-immunoreactive nerve fibers were stained, the grayscales of the groups which on day 1, 3, 5 after composite filling was significantly higher than control group (P<0.05), and there was no significant difference between control group and the experimental group on day 12 after composite filling(P>0.05).Conclusion The results show that the pain response induced by deep cavity composite filling may last for several days with peak response on day 1 to 3 and may alleviate as time pass-by.

    Objective To study the effect of proplis on the production of lactic acid from S.Mutans.Methods S.mutans and S.sobrinus were inoculated into the TPY culture mediums which contained 5 g/L or 1.25 g/L proplis extract in water, 1.562 5 g/L proplis extract in ethanol, chlorhexidine, and TPY culture medium as control group respectively. Having been cultured for 48 h, the lactic acid in each solution was examined and compared.Results There was no statistic difference between the groups of 5 g/L proplis extract in water, 1.562 5 g/L proplis extract in ethanol, and chlorhexidine, whereas the statsitc difference between the above three groups and the control group were significant, and there was also a significant difference between the 1.25 g/L proplis extract in water group and both chlorhexidine and control group.Conclusion similar to chlorhexidine, Beijing proplis extracts in water and ethanol apparently inhibit the lactic acid production by S. mutans and S. sobrinus.

    Objective To evaluate the clinical effects of minocycline ointment (Perioline) supplementary treatment on periodontitis resulted from fixed partial dentures.Methods 32 patients with periodontitis resulted from fixed partial dentures were enrolled in the study.Following supragingival scaling and subgingival scaling the patients were randomly divided into test group (80 sites) and control group (88 sits). The periodontal pockets were administered with either minocycline ointment in test group or saline solution in control group at baseline, week 2, week 3, and week 4.Clinical stores of plaque index (PLI), bleeding on probing (BOP), and probing depth (PD) were analyzed at baseline, week 4 and week 8.Results Both therapies resulted in significant improvements compared with baseline in terms of PLI, BOP and PD scores. Statistically significant differences were noted between the two groups, which suggested that the test group was superior to the control group.Conclusion Local administration of minocycline ointment (Perioline) in the treatment of periodontitis resulted from fixed partial dentures is safe and results in significant clinical improvement after supragingival and subgingival instrumentation.

    Objective To evaluate the clinical efficacy effects of flap surgery in combination with minocycline hydrochloride in treatment of chronic periodontitis.Methods Fifteen patients with chronic periodontitis were recruited into this study, with a total of 16 teeth in which probing depth of two sites was over 5 mm. The subjects were randomized into two groups. After scaling and root planning in three months, the experimental group received flap surgery in combination with minocycline hydrochloride, while the control group received flap surgery only. For both groups, the following indexes were recorded at the time of baseline (before root planning), 1 month and 3 months after the treatment: probing depth (PD), clinical attachment loss (CAL), plaque index (PLI) and sulcus bleeding index (SBI).Results At the time of baseline, there was no significant difference in the indexes between the two groups. At the time of 1 and 3 months after the treatment, both groups showed significant improvement in their examined indexes. The experiment group showed a greater gain in PD and CAL compared with the control group at 1 and 3 months, though there were no significantly difference in PD and CAL between the two groups at the time 1 and 3 months (P>0.05). However, There was significant difference in PLI and SBI between the two groups at the time 1 and 3 months (P<0.05).Conclusion Following both therapeutic modalities, there were marked clinical improvements for the chronic periodontitis at the time of both 1 and 3 months compared with baseline. And flap surgery in combination with minocycline hydrochloride is more effective.

    Objective To introduce different kinds of treatment for temporomandibular joint tubercle fracture.Methods According to fracture line in orbital axial CT, 35 cases of temporomandibular joint tubercle fracture were preoperatively devided into two types: sagital type and oblique type. Coronal incision was applied and temporomandibular joint tubercle fracture was reduced and fixed using one or two microscrews or microplates during operation. Orbital axial CT and coronal CT, three-dimension CT scan were performed in all cases preoperatively and postoperatively. Preoperative CT was compared with postoperative CT.Results Microplate fixation was applied for 24 cases, one microscrew fixation for 8 cases, two microscrews fixation for 3 cases. All the temporomandibular joint tubercle fractures were reduced correctly. Facial configuration of all the patients were restored and symmetrical.Conclusion The normal anterior and lateral of zygomatic process was resered after temporomandibular joint tubercle fracture reducing. Different methods of fixation were applied in accordance with exposure of fracture and types of fracture line.

    Objective To investigate the clinical effects of glass ionomer and fluoride varnishes in the treatment of root surface caries in senile patients.Methods A total of 214 teeth with root surface caries in 98 patients were randomized into two groups, with 114 teeth in group A and 100 teeth in group B. Group A was treated with glass ionomer and group B with fluoride varnishes. Six months and one year after treatment, the results of the two different methods were evaluated and compared.Results The success rates of the two methods were 77.88% and 75.82% respectively, there were no significantly different between glass ionomer and fluoride varnishes(P>0.05).Conclusion Both glass ionomer and fluoride varnishes were effective for the treatment of root surface caries in senile patients. Fluoride varnishes is especially indicative for the senile patients who are not tolerant toward drilling and filling.

    Objective To study the fatigue failure characteristics of direct and indirect resin composite restorations in-vitro by using a simulator to mimic chewing.Methods Thirty extracted human molars (prepared with standardized class Ⅱcavities, not bevelled) were randomly divided into three groups (n=10): two indirect resin composite restorations groups (cemented with PermaCem-Dual and ParaCore-Dual respectively) and a direct resin composite restorations group. All the specimens were fixed on a thermocycling and mechanical loading (TCML) chewing simulator. After thermocycling and dynamic mechanical loading ( 1 000, 5 ℃/55 ℃, 1.2×106, 50 N, 1.7 Hz), the specimens were immersed in a 1% Gentian violet solution for 24 h, rinsed, and then incised in a mesial-distal direction. Section surfaces were observed with an optical microscope.Results Compared with the direct composite group, both indirect composite groups showed a significant reduction in the incidence of micro-leakage of gingival margin (P<0.05), but there was no significant difference between the two indirect groups (P>0.05).Conclusion Within the limitations of this in-vitro study, indirect resin composite restoration can tolerate more fatigue failure than the direct-placement light-cured-only composite. The resin composite inlay technique is a promising method introduced to reduce the problem of shrinkage.

    Objective To study the cytocompatibility of several biomaterials such as polyhydroxybutyrate-polyhydroxyvalerate(PHBV) with human fibroblast cell and provide the better materials for vascular tissue engineering.Methods The fibroblast cell were cultured on the surface of several biomaterials in vitro, then the morphological characters were observed with fluorescence microscope and cell proliferation were detected with MTT assay. In addition, the biomaterials contact angle were measured.Results In all biomaterials, PHBV's contact angle was the smallest and it's cytocompatibility is the best.Conclusion PHBV has good cytocompatibility and could be the better biomaterial for the vascular tissue engineering.

    Objective To explore the relationship between connexin43 and ischemic arrhythmia during myocardial ischemia.Methods Barbiturate-anesthetized rats were subjected to a 30 min coronary artery occlusion. Rats were randomly assigned to control or ischemic preconditioning, and diazoxide groups. In two additional groups, rats received 5-hydroxydecanoic acid in the presence of ischemic preconditioning or diazoxide. Hemodynamics and arrhythmia were determined. Changes in phosphorylation of connexin43 were defined with quantitative immunoblotting.Results Ischemic preconditioning and diazoxide, a mitochondrial ATP sensitive potassium channel agonist induced phosphorylation of connexin43 and reduced arrhythmias as compared to control groups. The ATP sensitive potassium channel blocker 5-hydroxydecanoic acid prevented these effects in preconditioned and diazoxide hearts.Conclusion Ischemic preconditioning induces phosphorylation of connexin43 and electrical coupling by opening mitochondrial ATP sensitive potassium channel and then prevents the development of arrhythmias.

    Objective To study the effects of Jianpi Xiaozhang supplement on electro-mechanical activity in rat.Methods Colophony globules were administered to rats by gastrogavage and the gastric emptying rate was calculated by counting the number of colophony globules in stomach. The movements of stomach before and after gastrogavage were recording by using high sensitive stress plate transducer.Results The gastric emptying rates in rats administered with Jianpi Xiaozhang supplement in the dose of (5 g/kg, 2 g/kg and 0.5 g/kg) were quicker than that in control group(P<0.001,P<0.001,P<0.05). Jianpi Xiaozhang supplement in the dose of (5 g/kg, 2 g/kg and 0.5 g/kg)increased markedly the average amplitude of gastric movement and gastric slow wave.Conclusion Jianpi Xiaozhang supplement dose-relatedly enhanced gastric electro-mechanical activity.

    Objective The aim of this study was to investigate the efficacy of pentoxifylline in the treatment of NASH induced by high-fat diet.Methods A nonalcoholic steatohepatitis model was prepared by feeding rats with fat-rich diet for 12 weeks, then these rats were sacrificed. NASH was confirmed by liver histologic exam, then the rats were divided into 3 groups randomly, i.e. normal control group, model group and treatment group. The model group and treatment group were given high-fat diet and the normal group was given normal diet continuously. Furthermore,the rats of treatment group were given pentoxifylline16 mg/(kg·d) for 4 weeks. At the time of 16 weeks, the rats of three groups were sacrificed. The body weight, liver index(liver/body weight ratio), serum ALT,AST,TG,TC were measured and the liver histology of rats of all groups were examined with HE, Masson and oil red stain, and hepatic inflammation score was estimated.Results The body weight and liver index of model group were greatly higher than normal groups (P<0.01), but the body weigh and liver index of treatment group were increasingly dropped (P<0.01). The difference of the serum levels of ALT and AST between model group and normal group's was significant (P<0.01). Compared with normal group, The serum levels of AST in treatment group have significant difference (P<0.05). The serum level of TC of model group was increased significantly (P<0.05), the serum levels of TC,TG of treatment group were dropped, but there was no statistical significance and liver histology was showed hepatic inflammation score descended markedly (P<0.01).Conclusion Pentoxifylline could ameliorate the hepatic inflammation of rats with steatohepatitis induced by fat-rich diet and it suggests that pentoxifylline might be effective in treating rats with nonalcoholic steatohepatitis induced by high fat feeding.

    Objective Both protein kinase C (PKC) and nitric oxide synthase (NOS) play a key role in the dysfunctions of early diabetic retina, but their interaction remains unclear. To investigate the effect of PKC on NOS expression and nitric oxide (NO) production in the retina of 2-week diabetic rats.Methods PKC inhibitor GF109203X was injected intravitreally after 2 weeks of streptozotocin-induced diabetes. PKC activity was measured by ELISA. NOS mRNA was analyzed by semi-quantitative RT-PCR. NO was determined indirectly as nitrite and nitrate.Results Increases of PKC activity, neuronal NOS (nNOS) mRNA expression and NO were found in the retina of diabetic rats compared with normal rats. Inducible NOS (iNOS) expression was slightly upregulated in diabetic retinas, but without any significant difference. Upregulation of nNOS mRNA was inhibited 12 h and 24 h after GF109203X was injected. NO also decreased 24 h after the injection compared with diabetic rats.Conclusion The results suggest that nNOS upregulation might be a result of PKC activation in the retina of early diabetes. This might be one of the mechanisms of retinal neuronal dysfunctions and vascular hyperpermeability. PKC inhibitor could reverse the abnormal increases of nNOS expression and NO.

    Objective To investigate the protective effects of mast cells on pancreas of rats with severe acute pancreatitis(SAP).Methods Ketotifen was injected intraperitonealey 30 min before or after SAP model, 3 h and 6 h after operation, pancreas was removed to make pathologic score, to count the number of MC by the method of toluidine blue staining and immunohistochemistry, to measure the MPO activation, the activation level of tryptase by western blot, and compared with those of SAP rats without ketotifen treatment.Results Pretreatment or early using of ketotifen after SAP can decrease the damage of pancreas, decrease the MC number and the level of MPO and tryptase.Conclusion MCs are activated in the early stage of SAP,and induce damage to pancreas. Pretreatment with or early using of ketotifen can decrease the severity of pancreas damage in SAP.

    Objective To investigate the changes of enzymes activities of jejunum in rats with severe acute pancreatitis(SAP).Methods Rats were divided randomly into 3 groups:sham operation group (SO), SAP group, and treatment group with Gln (SAP+Gln).SAP model was reproduced by retrograde infusion of sodium taurocholate solution into the biliopancreatic duct. In addition, the SAP+Gln group was feeded with glutamine after operation. Tissues of jejunum were taken at 24 h, 48 h and 72 h respectively after operation, and the activities of alkaline phosphatase, ATPase, cytochrome oxidase, succinate dehydrogenase were studied using enzyme-histochemistry staining.Results The activities of ALP,ATPase,CCO,SDH decreased significantly in SAP group,while Gln could increase them significantly.Conclusion It suggest that the intestinal function was impaired in SAP.Gln could protect intestinal function.

    Objective To study the Antiangiogenic potency of BAI1 in vivo and the therapeutic effect on human glioblastoma. Method The COS-TPC method was used to construct recombinant adenovirus carrying the human BAI1 cDNA or LacZ(Mock). BAI1 mRNA and protein were detected using Northern Blotting and Western Blotting respectively. Dorsal skinfold transparent chamber analysis was used to evaluate the antiangiogenesis effect of BAI1. Observations were performed using intravital microscopy and a S-VHS video tape recorder. Immunohistochemical staining using anti-von Willebrand factor antibody was used to show the stained endothelia inside tumor. Blood vessels were counted under microscope. U373MG cells were inoculated into mice subcutaneously. After the tumors grew up to a size of 0.5~1.0 cm in diameter. AdeBAI1, or AdeMock, or Medium were injected intra-tumorly, this injection was repeated 5 times every three days. The tumor size was observed and recorded every other day. Result The results of Northern Blotting and Western Blotting showed that BAI1 mRNA and protein was expressed only in those transduced with AdeBAI1. In mice injected with DMEM or AdeMock tumors, blood vessels were infiltrated into the tumor tissue. On the other hand, angiogenesis was completely inhibited in dorsal skinfold chamber of SCID mice transplanted with AdBAI1-transduced cells at day 12 after the transplantation. Immunohistochemical staining using anti-von Willebrand factor antibody showed decreased vascularization was evident in the xenografts of AdBAI1-transduced U373MG cells compared with AdeMock-transduced or nontransduced U373MG cells in SCID mice. The mean tumor sizes of SCID mice treated with AdeBAI1 were significantly small than that of those treated with AdeMock, or Medium.Conclusion BAI1 has antiangiogenesis effect. Intratumor injection of AdeBAI1 can inhibit the tumor growth. BAI1 was one of the potent candidate genes for gene therapy of brain tumors, particularly human glioblastomas.

    Objective To investigate the pathogenetic mechanism of diabetic encephalopathy..Methods The non-insulin-dependent diabetes mellitus (NIDDM) animal models, KK-Ay mice, were used. Their cognitive performance and brain structure were studied. The activity of choline acetyltransferase (ChAT) was assayed with 3 H-CoA as the substrate and the binding activity of radiolabelled 3 H-QNB for acetylcholine muscarinic receptors (Mreceptors) was investigated in the cerebral cortex and hippocampus of KK-Ay mice..Results Cognitive impairment and changes of brain structure of NIDDM mice were found. In the cerebral cortex and hippocampus of KK-Ay mice, the activity of choline acetyltransferase (ChAT) was significantly reduced at 12 weeks of duration compared with that of age-matched controls. The binding activity of 3 H-QNB for M receptor in the diabetic mice was not significantly different from that of the controls..Conclusion The acetylcholine system deficits are closely related to the cognitive impairment of diabetic mice.

    Objective To investigate the epidemic of HBV genotypes among HBV-infected persons in Beijing.Methods Ten outer and inner primers were designed on the basis of the conserved nature of nucleotide sequences in regions of the Pre-S1 through S genes, in which 8 inner primers were divided into mix A and B to amplify HBV OF genotype A, B,C and D, E, F respectively. Genotypes of HBV were determined directly by the size of PCR products.Results The HBV DNA in 728 sera samples collected from HBV-infected patients with different outcome were tested and genotyped by this nested PCR. 276 samples (37.9%) were HBV DNA positive. 5.8%(14/243) self-limited HBV-infected patients were HBV-DNA positive, while chronic asymptomatic HBV carriers 42.6%(113/265), chronic viral hepatitis 67.7%(149/220)respectively. There was significant difference between the chronic viral hepatitis and the other two groups. Among the 276 HBV DNA positive samples, the percentage of genotype C was 76.8%, type B 21.7%, mixed genotype B and genotype C 1.5%, while none of genotype A,D,E and F had been found.Conclusion The HBV genotypes prevailed among HBV-infected persons in Beijing are genotype B and C, and the major genotype is genotype C. There is no statistical difference between the groups.

    CYP11B2(-344T/C) in 345 hypertensive patients (EH) and 156 normotensive controls (NE). Clinic blood pressure, biochemical indexes, the indexes of renin angiotensin-aldosterone system were also detected.Results The frequencies of CC+CT genotype of aldosterone synthase gene CYP11B2(-344T/C) polymorphism among EH was significantly higher than that among NE(P<0.05), the C allele of CYP11B2(-344T/C) polymorphism among EH was significantly higher than that among NE(P<0.01). ALD and ALD/REN of CC+CT genotype of CYP11B2(-344T/C) polymorphism were significantly higher than TT genotype(P<0.05). There were no significant difference in REN and ATⅡ.Conclusion The CYP11B2(-344T/C) polymorphism could significantly influence the RAAS in Han Nationality essential hypertensive patients in Beijing.

    Objective To analyse and summarize the clinical characteristics of hyperlipidemic pancreatitis.Methods A retrospective research was carried out to compare clinical characteristics on 979 patients which admitted to Xuanwu hospital from Jan. 1988 to Dec. 2004. 613 patients had acute gallstone pancreatitis, 90 patients had hyperlipidemic pancreatitis.Results The levels of blood and urine amylase in HLP was lower than that of acute gallstone pancreatitis. HLP had higher incidence of circulation failure, respiration failure, kidney failure and lung infection.Conclusion In the diagnosis of HLP, it should be noticed that the elevation of blood and urine amylase is not frequently remarkable. In the course of HLP'therapy, it is important to prevent and treat multi-organ failure and lung infection.

    Objective To investigate the changes of the blood lipid level in patients with hypertension and in patients hyperuricemia or in patients with hypertension combined with hyperuricemia respectively.Methods Blood uric acid (UA), total cholesterol (TC), triglyceride (TG) and high density lipoprotein cholesterol (HDL-C) were measured in one hundred and three patients with hypertension, thirty nine patients with hypertension complicating with hyperuricemia caused by diuretic therapy, one hundred and twenty eight healthy subjects as control and seventy four patients with hyperuricemia and normal blood pressure. The volume of low density lipoprotein cholesterol (LDL-C)were calculated in all group.Results The incidence of increased serum lipid was higher in hyperuricemia group, hypertension group, hypertension combined with hyperuricemia group than in control group. Hypertension and hyperuricemia either had effects on serum lipid levels.Conclusion Hyperuricemia is correlated with hypercholesteremia closely. Hypertensive patients complicating with hyperuricemia caused by diuretic therapy could be coincident with disorder of blood lipid as well.

    Objective To investigate aspirin resistance (AR) in the aspirin-treated patients with coronary artery disease, and further to evaluate the clinical characteristics of them.Methods We prospectively enrolled 102 patients with coronary artery disease on aspirin ( 100 mg/d for ≥2 weeks ) with no other anti-platelet agents. We tested for AR by optical platelet aggregation (PAG) using ADP and AA plasma and urine thromboxane B2 (TXB2) concentration were measured by TXB2 kits. AR was defined as a most PAG of ≥50% with ADP and ≥30% with AA.Results Of the patients studied, 13 (12.7%) were AR. The PAG of AR group was higher than AS group (P<0.01). Urine TXB2 of AR group was higher than AS group. Plasma TXB2 was no statistical diversity between groups. The hs-CRP of group with high TXB2 is higher than the hs-CRP of group with low TXB2.Conclusion This study detect 12.7% AR in 102 cases aspirin-treated patients with coronary artery disease.

    Objective To identify risk factors in patients with nonvalvular atrial fibrillation(NVAF).Methods Retrospective analysis of our hospital records were performed from patients with primary diagnosis of AF, discharged from Jun. 2002 to Sep. 2003.Results The data of the patients with stroke in NVAF (n=159) was compared with those in NVAF without stroke ( n= 279). The effect of each variable on stroke was assessed with a logistic regression analysis. A total of 438 cases in NVAF were enrolled. Mean age was 69.4 years old. Univariate analysis showed that significant risk factors for stroke in patients with AF were age>75, hypertension, diabetes mellitus, history of congestive heart failure; old age, increased serum total cholesterol and glucose, high systolic and diastolic blood pressure, left ventricular dysfunction. In multivariate analysis, old age, history of hypertension, left ventricular dysfunction was independently associated with stroke. It was 66.5% of our patients who received antithrombotic therapy with dominated use of antiplatelet agents.Conclusion Our analysis suggests that old age, history of hypertension, left ventricular dysfunction (EF<50%)are independent risk factors, which should be considered when decision of long-term anticoagulation therapy to prevent stroke in NVAF is to be made. It was 66.5% of our patients who received antithrombotic therapy with dominated use of antiplatelet agents. Therefore it is advisable to enforce the control of AF.

    Objective To determine the relationship between H. pylori infection and the anatomic distribution of chronic atrophic gastritis, a cross-sectional study was carried out in a population at high risk of gastric cancer.Methods From 1989 to 1990, 3 400 residents aged 3564 in Linqu County, Shandong Province in China received gastroscopic examinations with biopsies taken from 7 standard sites in the stomach and the status of H. pylori infection of each participant was determined by ELSIA. The relationship between H. pylori infection status and the extension of CAG in gastric mucosa was analyzed.Results A total of 1 428 subjects were diagnosed of CAG and 801(56.1%) of those with CAG were serum H. pylori antibody positive. The proportion of CAG with more extensive distribution among H. pylori infection positive was significantly higher than the subjects with H. pylori infection negative (52.2% vs 36.2%, OR 3.2, 95% CI: 2.4~4.4, sex and age adjusted).Conclusion H. pylori infection was more likely to play an important role in on the progression of CAG from local to extensive distribution in the gastric mucosa. This population-based study provides further evidence that H. pylori infection is an important risk factor for the progression of CAG.

    Objective To compare the effects of sevoflurane with different concentrations on the electroencephalogram(EEG) in epileptic or non-epileptic patients and investigate the suitable concentration of sevoflurane for epilepsy surgery.Methods The changes of epidural EEG were observed during anesthesia with 0.7, 1.0, 1.3 and 1.5 MAC sevoflurane in 20 epileptic patients undergoing epilepsy surgery and in 15 non-epileptic patients of neurosurgery.Results The frequencies of spike (or sharp) waves were similar at 0.7, 1.0 and 1.3 MAC, and increased significantly at 1.5 MAC in epileptic patients. No spike waves appeared at 0.7, 1.0 and 1.3 MAC and five cases showed spike bursts at 1.5 MAC in non-epileptic patients. The frequencies of α and β waves decreased and δ wave increased with the increases of sevoflurane concentration in both epileptic and non-epileptic patients.Conclusion The maintenance of anesthesia with 0.7~1.3 MAC sevoflurane is suitable for the epilepsy surgery with intraope-rative EEG monitoring, which does not influence the activity of spike bursts.

    Objective To investigate the profile of anesthesia with midazolam and propofol.Methods 90 patients were randomly allocated into three groups. In group A, patients were induced with midazolam 0.2 mg·kg-1 and maintained with 0.15 mg·kg-1 ·h-1. In group B, propofol 1~2 mg·kg-1 was used for induction, and 2~4 mg·kg-1 ·h-1 for maintenance. In group C, midazolam 0.1 mg·kg-1 and propofol 0.51 mg·kg-1 were used for induction, and midazolam 0.1 mg·kg-1 ·h-1 and propofol 1~2 mg·kg-1 ·h-1 were infused for maintenance. Fentanyl 1~3 μg·kg-1 and vecuronium were injected intravenously and 1%~1.5% isoflurane was inhaled as needed. SBP, DBP, HR and SpO2 were monitored continuously during anesthesia. The recovery was observed.Results SBP, DBP, HR, SpO2 of every stage were compared with those of preanesthesia in each group, it was found that SBP, DBP in group C had no difference (P>0.05), and had significant difference in group A and B (P<0.05). The patients awakened (34.44±12.35) min after operation with 6 anxiety, 11 nausea, 9 vomiting and no case dream during the first 24 h after surgery in group A, The patients awakened fast (6.56±5.74) min with 9 anxiety, 7 dream, 13 nausea, 11 vomiting and 16 pain during the first 24 h after the operations in group B, The patients opened their eyes (14.64±9.50) min after surgery with 5 anxiety, 2 dream, 8 nausea, 4 vomiting and 10 pain during the first 24 h after operations in group C.Conclusion The combination of midazolam and propofol is a suitable method for general anesthesia.

    Objective To explore the value and safety of neuronavigation used in the surgery for anterior cerebral artery aneurysms.Methods A retrospective review of 12 patients with anterior cerebral artery aneurysms underwent neurosurgical operation under the guidance of an image-guided system by Sofamor-Danek company was made.Results There were no postoperative neurological complications. No operative death occurred. The aneurysms and the important anatomic structures were accurately located.Conclusion The neuronavigation system is very helpful for anterior cerebral artery aneurysms surgery. Its accuracy of location is very important in protecting normal brain tissues.

    Objective To evaluate the clinical manifestations of solid-pseudopapillary tumor of the pancreas (SPT) and examine the diagnosis, treatment and prognosis of this rare disease.Methods We retrospectively reviewed a series of eight patients with SPT admitted in our hospital from April 1980 to July 2005.Results All the patients are female with mean age of 24.5 years (range 18 to 34 years) were diagnosed with SPT at our institution. Clinical presentation included a palpable abdominal mass in four patients and abdominal distention in another two. Two patients were asymptomatic; their tumors were found incidentally on abdominal sonographic examination for other reasons. Solid and solid-cystic masses of low echo were found in pancreas in ultrasonic examinations. CT scan found masses of Low density in pancreas, while irregular enhancement appeared in the circumference or parenchyma of all tumors in enhanced CT scan sequences. Tumor markers in patients' serum were all negative. The lesions were located in the body and tail in three cases and in the head of the pancreas in five. The mean diameter of the tumors in the eight patients was 6.2 cm (range 4 to 8 cm). Pancreaticoduodenectomy were performed in 5 cases. Three patients underwent distal pancreatectomy and spleen resection. None of the patients received adjuvant therapy. The mean follow up was 7.25 years (range 3 to 25 years). No evidence of relapses and metastasis in these cases was found.Conclusion Solid-pseudopapillary tumor of pancreas is a rare tumor with low malignant potential, affecting primarily young women. Surgical resection is generally curative and the prognosis is excellent.

    Objective To evaluate the minimum dosage of efficiency in diagnostic radiation for intracranial germinoma to reduce the patients' unnecessary radiation.Methods Retrospective analysis were performed in 75 germinoma patients with 104 lesions totally that were clinically verified by radiotherapy, including 51 patients with single lesion, 19 patients with two lesions and 5 patients with three lesions. The response case number were analyzed by chi-squire examination at the radiation dosage of 10 Gy and 20 Gy.Results Statistical analysis showed no difference in the number of responsive patients between 10 Gy and 20 Gy radiation dosage.Conclusion For intracranial germinoma, 10 Gy is the preliminary minimum dosage of diagnostic radiation to confirm the suspected germinoma.

    Objective To study on repair of articular cartilage defects with combined bone morphogenetic protein(BMP)/ basic fibroblast growth factor (bFGF) material in order to supply experimental basis for repairing cartilage defects biologically.Methods The rabbit were chosen and full thick defects of cartilage were made in intercondylar fovea of the femur, the defects were repaired with different materials(rhBMP2/PVP,bFGF/PVP,rhBMP2/bFGF/PVP,PVP), and the results were observed with histology and electron microscope.Results 8 weeks after operation,the defects of cartilage had almost repaired in the rhBMP2/bFGF/PVP group,12 weeks after operation, the cartilage were smoother,the results of other groups were not so good as this group.Conclusion BMP had the ability to induce cartilage formation,combined BMP/bFGF material increased cartilage cell proliferation,and had the better result than using BMP alone,combined BMP/bFGF material was good for repair of articular cartilage defects.

    Objective To investigate the clinical characteristics, early diagnosis, preventive and therapeutic methods, and the effects on maternal and infant health of encephalopathy during pregnancy.Methods The clinical materials of 11 cases with pregnancy associated with encephalopathy in Tiantan hospital were analysed retrospectively.Results Pregnancy can accelerate the growth or rupture of cerebroma and encephalic angioma. Craniotomy was necessary as soon as possible. Caesarean section was the suitable delivery modus. We should focus on prevention and treatment of post-eclampsia cerebral infarction.Conclusion The high rate of misdiagnosis, quick development and high rate of death account for characteristics of encephalopathy during pregnancy. Regular antenatal examination, early diagnosis and just-in-time treatment are critical to improve the prognosis.

    Objective To investigate the hemodynamic changes of the pregnancy failure by Doppler ultrasound.Methods 273 cases were examined with ultrasound, there were 156 cases of normal early pregnancy, 52 cases pregnancy failure and 65 cases anembryonic pregnancy. The bilateral arteries of uterus and ovary were measured by trans abdominal color and pulsed Doppler. The data of the blood flow were analysed by statistically.Results The measurement of the pulse index(PI), the resistant index(RI) and the ratio between the peak of systolic velocity and the end of diastolic velocity(S/D) of dominant and non-dominant uterus artery in pregnancy failure were much lower than that of normal early pregnancy. There was significant statistic difference in the PI, RI and S/D between them. And there were similar ities between the pregnancy failure and anembryonic pregnancy, with no statistic difference between them. No significant difference was found between the pregnancy failure and normal early pregnancy in terms of ovarial artery measurement.Conclusion The change of PI and RI in uterus artery could be used to evaluate the outcome of the pregnancy. When the PI and RI of the dominant uterus artery are lower than 1.80 and 0.77 respectively, the risk of the pragnancy failure is higher than normal.

    Objective To investigate the mechanism of Liang Xue Huo Xue Tang in the treatment of psoriasis vulgaris.Methods 45 cases of psoriasis vulgaris classified as blood heat type according to TCM differentiation-types were studied. Serum levels of VEGF in patients and normal controls were measured by enzyme-linked immunosorbent assay (ELISA)before and 2, 4 weeks after treatment.Results Compared with the data of normal controls, the serum levels of VEGF in patients were significantly increased (P<0.001), While a significant difference of the serum levels of VEGF in patients was observed between the per and post treatment (P<0.01). At the end of the 4 weeks treatment, the serum levels of VEGF in patients were returned to normal range.Conclusion These results indicated that Liang Xue Huo Xue Tang could reduced the serum levels of VEGF in patients with psoriasis vulgaris, this study suggested that suppression of angiogenesis may be one of the mechanism of Liang Xue Huo Xue Tang in the treatment of psoriasis vulgaris.

    Objective It was reported that H.pylori is one of the high risk factors in chronic gastritis, ulcer, and gastric cancer, The aim of this study is to provide a useful method for detecting H. pylori.Methods In 150 patients H.pylori was detected with ureasetesting, warthin-starry histology, ELISA technique, and 13 C-breathtest-instructions.Results The sensitivities of the four methods are: ureasetesting (84.6%), histology (95.6%), 13 C-breathtest-instructions (97.8%), ELISA technique(89.0%). The speciticities of the four methods are urease testing (77.2%), histology (78.9%), 13 C-breathtest-instructions (88.2%), ELISA technique(75.6%) respectively.Conclusion Histology are the method with best accuracy for the detection of H.pylori.

    Objective To compare the clinical characteristic between senile depression and non-senile depression.Methods 43 patients who met the DSM-Ⅳcriteria geriatric depression and 45 patients who met the DSM-Ⅳcriteria general depression were included in this study to compare the clinical characteristics of the two groups.All patients were evaluated with the HAMD, HAMA scales.Results In old group, the score of agitation and hypochondriasis was significantly higher than the non-elder group(P<0.01); and in elder group, the cardiovascular systemic symptoms has largest proportion in all somatic symptoms, and that is significantly difference from the non-elder group(P<0.01).The proportion of gastrointestinal symptom was largest in all somatic symptoms in non-elder group. At the same time, the score of anxiety /somatization, cognition disorders and sleeping disorders in elderly group were significantly higher than those in non-elder group(P<0.01,P<0.05).Conclusion Besides the classical symptoms of depression, hypochondriasis, agitation, somatization and depressive pseudodementia were the features of elderly depression.

    Objective To investigate the relationship between abnormal fetal heart and the neonatal prognosis during the second birth process.Methods 109 cases of neonatal asphyxia delivery in our hospital were reviewed.Results 97 cases were mild asphyxia and 12 cases severe asphyxia. Abnormal FHR devided into mild VD 20 cases, medium VD 28 cases, severe VD 15 cases, LD 1 case, VD with TDr 9 cases, marked bradycardia 16 cases, and VD with PD 20 cases.Conclusion There were significant difference between abnormal FHR and neonatal asphyxia ( P< 0.01). The second birth process should be pay more attention.
