    Effects of Brain Tissue Extracts of Hypoxia Preconditioned Mice on PC12 Cells

    To study the protective action of cerebral homogenate extracts on in vitro preparation, the present experiment was conducted on a hypoxic model of PC12 cells. LDH and MTT assay were used to measure changes of the cells′activity following incubation with brain tissue extracts taken from hypoxia preconditioned mice for 24, 48 and 72 h. The cells′activity was found to be significantly increased when the cells were incubated with the extracts containing protein and no significant change was seen when the cells were incubated with protein free extracts. The result indicates that protein like neuroprotective substance(s)is developed in the brain of hypoxia preconditioned mice.

    The biological properties of cultured human dental pulp cells were investigated in the study. Dental pulp was isolated from human young permanent teeth and cultured in vitro routinely. Fibroblasts usually appeared to migrate from primary explants of dental pulp after 7 to 10 d. Thirty days after explantation, the migrating cells formed confluent monolayers and were subcultured. With the techniques of immunocytochemistry, the pulp cells showed high levers of alkaline phosphatease activity. In addition to the production of type I collagen, these cells also synthesized fibronectin. With continued culture, the pulp cells formed nodules 10 to 15 d after being dispersed in the flask. The matrix formed in these nodules strained strongly with Von Kossa. This research provides a useful system for studying the physiology and calcification of pulp tissues.

    The experimemt observed effect of flaxseed on Wistar rats′ serum triglyceride(TG), total cholesterol(TC), high density lipoprotein cholesterol(HDL-C), low density lipoprotein cholesterol(LDL-C), Apo B and atherosclerosis index(AI). The rats were divided into a lard group, three flaxseed groups(low, middle and high level)and a basal diet group. Thirty two days later, significant decrease in serum TC and LDL-C were found in the flaxseed groups(low, middle and high level)as compared with the lard group(P<0.05), significant decrease in serum TG and significant increase in serum Apo B, HDL-C/TC and AI were found in the flaxseed groups(middle and high level)as compared with the lard group(P<0.05), but there was no difference in HDL-C between flaxseed group and lard group(P>0.05). The experiment indicates that flaxseed rich in α linolenic acid can be used to prevent and lower blood lipid, and deserves further study and development.

    Protective effects of baicalin on myocardium ischemia-repurfusion injury were studied in rats. Wistar rats were divided into 5 groups: pseudo operation; ischemia reperfusion operation; ischemia-reperfusion with baicalin 20 mg/kg and 80 mg/kg; ischemic-reperfusion with Verapamil 1 mg/kg. The results showed that baicalin reduced frequency of arrhythmia, contents of CK and MDA, and increased activities of SOD and GSH-PX. The results suggest that baicalin possesses protective effects on myocardium ischemia reperfusion injury in rats and its mechanism is related to inhibition of lipid peroxidation.

    The polymorphism of G/A 1947 in exon 4 of COMT gene was analyzed in a population of 53 randomly selected Chinese individuals by polymerase chain reaction restriction fragment length polymorphism(PCR-RFLP). The frequency of A allele was 0.28. The index of homozygous G/G、G/A and A/A were 0.51, 0.42 and 0.07 respectively. The C1883/G and G1966/T polymorphism were found in a Chinese by genetic analyzer technique.

    To study transfusion transmitted virus(TTV)infection from liver tissues, double-strand and single-strand probes were labelled with digoxigenin to detect 56 liver samples by in situ hybridization and by symmetrical and unsymmetrical polymerase chain reaction(PCR). TTV DNA was found in 14 of 51 patients with non hepatitis A and non-hepatitis G(27.5%). Five liver samples without hepatitis were negative. TTV DNA was mainly observed in nuclei of hepatocytes and distributed in the tissues with acute, chronic and severe types of hepatitis. The hybridization result by double-strand probe was consistent with TTV genome strand hybridization by single-strand probe. In 2 of 6 liver samples were present both TTV genome strand and complementary strand. This study indicates that TTV is a novel virus which results in hapetitis and the virus replicates in liver tissue of the patients.
    The 4G/5G Polymorphism of the Plasminogen Activator Inhibitor-1 Gene in Patients with Coronary Heart Disease

    WT5BZ]To investigate the relationship between 4G/5G polymorphism of the plasminogen activator inhibitor-1(PAI-1)gene and coronary heart disease(CHD), genotype at this polymorphism was determined in 123 cases with CHD and 172 controls by polymerase chain reaction using the allele specific primers. Plasma PAI-1 antigen level, PAI activity, cholesterol and triglyceride were measured and body mass index was calculated from all the subjects. The group of CHD had a high number of homozygotes for the deleted allele(4G/4G)of the PAI-1 gene compared with the control(47.2% vs 22.1%, P< 0.05). Mean plasma PAI-1 antigen level and PAI activity varied significantly among the 3 genotypes, being higher in 4G/4G subjects than in 5G/5G subjects 〔(40.87±0.99)μg/L〕 vs 〔(38.14±1.0)μg/L〕, 〔(750±350)U/L〕 vs(650±270)U/L, P<0.05). The PAI-1 4G/5G polymorphism correlated with PAI-1 antigen level to some extent accounts for the development of CHD.

    The study is to investigate whether the insertion/deletion polymorphism of angiotensin 1 converting enzyme(ACE)gene is associated with Type 2 diabetes mellitus without cardiovascular complications(DM2)and that with myocardial infarction(MI)in Chinese, and to ascertain the interaction of serum ACE levels and ACE gene polymorphism to the development of the associated diseases. ACE genotypes of 82 cases with MI, 86 with DM2 and 84 controls were determined by polymerase chain reaction(PCR). Serum ACE levels were measured with ultraviolet photometry. The increased frequencies of D allele(0.52)and DD genotype(0.32)were respectively observed in 82 subjects with MI compared with 84 control subjects(0.36,0.15 respectively, P<0.05). The odds ratio associated with DD/II genotype was 3.01(95%CI 1.28~7.04, P<0.01)for MI. Individuals with DD genotype had the highest serum ACE in all groups, and with II genotype the lowest levels. There was no difference in allele distribution between 86 DM2 patients and the controls. ACE gene polymorphism is associated with serum ACE levels(r= 0.65, P<0.01). Deletion polymorphism of the ACE gene contributes to the development of myocardial infarction and is a risky factor for MI in Chinese type 2 diabetes.

    For investigating and comparing the effects of fosinopril, losartan and nifedipine GITS on protective functions for endothelium in patients with essential hypertension(EH), patients with mild to moderate EH and healthy controls were studied. Patients with EH were randomly divided into three groups and were treated with Nifedipine GITS, Fosinopril or Losartan separately. The plasma levels of ET, NO, GMP140, vWF were compared before and after 4-week treatment. Plasma levels of ET, NO, vWF, GMP140 were measured with radioimmunoassay, enzymatic assay or ELISA as indexes of endothelial functions. The results showed that the antihypertensive effects of three medicines had no statistical differences. Plasma levels of ET, GMP140, vWF were significantly higher and the NO/ET was lower in EH patients than those in controls. Plasma levels of ET and vWF were decreased and the levels of NO and NO/ET were increased deeply at the end of 4-week treatment(P<0.05, P< 0.01). Plasma level of GMP140 in group N was declined significantly(P<0.05). It is suggested that all three antihypertensive drugs can significantly and safely decrease blood pressure, and protect vessel endothelial function in early stage of hypertension. Nifedipine GITS is better than others in GMP140, Losartan is better than others in drug tolerance.

    To investigate the effects of bisoprolol on heart function in old myocardial infarction(OMI), 35 patients with OMI received bisoprolol 2.5 mg per day for two months. Doppler echocardiographic measurements of left ventricular structure, transmitral flow velocity pattern, blood pressure and heart rate were monitored before and after 1 and 2 months therapy. Bisoprolol led to reduction in blood pressure and heart rate 〔(20.08/10.64±3.33/2.26)vs(19.42/10.37±2.79/1.86)kPa P<0.01,(70.66±12.86)vs(64.31±9.84)bits/min P<0.01〕, and E/A increased significantly 〔(0.85±0.18)vs(1.14±0.35)P<0.01〕. No significant affection was found on left ventracular systolic function. Bisoprolol reduces blood pressure and heart rate, improves diastolic function, hence it would be valuable in treatment of OMI.

    One hundred and fourteen patients with Acute Virus Myocarditis(AVM)were reported from Jan 1977 to 1999, male: 61 patients, female: 53 patients, age: from 16 to 48 years old, average age:(38.6±2.1)years old. According to their clinical symptoms, AVM patients were divided into three types: severe type, cardiac arrhythmia type and single ST-T wave shift type. AVM patients were tested by clinical sign, ultrasound cardiograph, serum enzyme test and serum virus test. The clinical symptoms varied from each patients with AVM. There were severe injuries of other organs in some patiernts with AVM, and left ventricular inner diameter enlarged in a few patients with AVM, and then developed into Dilated Cardiomyopathy. Serum enzyme increased for a long time without enzyme peak changes. Positive rate was very low in serum virus test in patients with AVM. Clinical diagnose of AVM is mainly on the basis of patient's clinical signs and injuries of the myocardium.

    To elucidate the effects on renal hypertension and progressive renal disease of endothelin 1 in patients of chronic glomerulonephritis, plasma endothelin 1(PET-1), urinary excretion rate of endothelin 1(UET-1), creatinine clearance(Ccr)and urinary excretion rate of β2- microglobulin(β-m)in 54 patients with chronic glomerulonphritis and those in 20 normal control were measured. The Results indicated the level of PET-1 and UET-1 in all patients was markedly elevated. There was a positive correlation between PET-1 or UET-1 and mean ertery pressure(MAP)and a negative relation between the former and β-m. Correlation between PET-1 or UET-1 and Ccr were found. Endothelin plays an important role in developing renal hypertension and in porgressive renal disease and also UET-1 is a sensitive marker of renal tubular damage.

    To determine the effects of urokinase and ticlopidine on blood viscosity parameters in the elderly patients. Fouty-six patients were randomly divided into two groups. Twenty-four patients were treated by vein with urokinase at dose of 250 000 U/d, 22 patients were treated with ticlopidine at dose of 0.25 g/d. And blood viscosity paramoters were measured. Both urokinase and ticlopidine could decrease blood viscosity, but have different effect on blood viscosity parameters. The two drugs have different effects on treating blood hyperviscosity. They should be used according to blood viscosity parameters.

    To identify the clinical significance of cardiac troponin T(cTnT)in asymptomatic patients with chronic renal failure(CRF), vein blood was drawn from patients with CRF of no hemodialysis(HD), maintenance HD, the skeletomuscular damage, the acute myocardial infarction and CRF with heart failure patients in proper order. Fullautomatic ELISA assay was used to determine cTnT serum levels, assay serum cretinine(Cr), creatine phosphate kinase-MB(CK-MB), lactic dehydrogenase(LDH)and hydroxybutyric acid dehydrogenase(HBD). The results is that during acute myocardial infarction, the mass density of cTnT increased significantly as a result of myocardial infarction. Blood cTnT was elevated in patients of CRF with heart failure. Blood cTnT fell to normal as soon as the heart failure was cured. But cTnT was not significantly elevated in asymptomatic patients with CRF. There was no clinical specificity for cTnT assay in asymptomatic patients with CRF.
    Relationship between Liver Fibrosis Stages and the Diameters of Wide Main Portal Vein and Spleen Vein and Splenectasis and Blood Tests

    In order to study the relationship between liver fibrosis stages and the diameters of main portal vein(MPV), spleen vein(SPV), splenectasis and blood tests, 1 005 patients with chronic hepatitis were divided into groups S0 to S4 according to liver fibrosis stages. The MPV and SPV diameters and splenectasis were measured by ultrasonography. The WBC, Hb, PLT, ALB, γ-G, PTA were tested by routinel method. From S0 to S4, the rates of wide MPV and SPV diameters and splenectasis increased gradually. The rates of wide MPV and SPV diameters were not significantly different(P>0.05)among the groups while the rates of splenectasis were significantly different(P<0.05)among the groups. Splenectasis occured earlier. The rate of splenectasis was 28.8% in S1 and it was significantly different between S1 and S0(P<0.05). The rate of wide MPV diameters was only 6.2% in S1. In S4, the rates of splenectasis and wide SPV, MPV diameters were 43.3%, 36.7%, 16.7% respectively. In blood tests, WBC, Hb, PLT, ALB, PTA were gradually decreased and γ-G was gradually increased from group S0 to S4 successively. In group S4, WBC, Hb, PLT were still normal, while ALB(<33.8 g/L), PTA(< 70.5%), γ-G(23.9%)were abnormal. Conclusion: The appearance of splenectasis is earlier and it may be more sensitive than SPV and MPV. If the effect of necroinflammatory activity of liver can be excluded, the earlier liver fibrosis may be consided under the conditions ALB< 33.8 g/L, PTA< 70.5%, γ-G> 23.9%.
    TT Virus Infection and Autoimmune Diseases in Non-A, Non-G Hepatitis

    All 48 screened sera were unknown etiological hepatitis. Four kinds of autoantibodies, including ANA,AMA SMA and anti-liver/kidney microsomal(LKM), were detected by indirect immuneofluorescence. TTV DNA were tested by nest PCR. Twenty-two of 48 patients(45.8%)were found with autoantibodies, 15(31.3%)with ANA, 5(10.4%)with AMA, 4(8.3%)with SMA, none with LKM. Seven of 48 patients(14.6%)were finally diagnosed as sufferering from autoimmune diseases: 3 had antoimmune hepatitis; 2 primary billiary cirrhosis and 2 primary Sjogren′s syndrome. M∶F=1∶6. Fourteen of 36(38.9%)were positive for TTV DNA. M∶F=2∶1. Three were also found to have autoantibodies in their sera. Autoimmune diseases and TTV infection prescented in Non-A, Non-G hepatitis. Severe damage in liver function could be seen in TTV patients, meanwhile autoimmune reaction was produced in some of them.

    Human cadaveric spines were used to determine sagittal horizontal stability of spondylolysis. Eleven FSU got from six fresh cadaveric human spines were tested for intact FSU and spondylolysis by using shear force. Spondylolysis was significantly less stable than the intact spine(P<0.01). Spondylolysis makes lumbar spine unstable, reduces its ability to withstand the shear force, and mades it prone to slipper.

    Eighty-four women with AUB received hysteroscopic transcervical resection of the endometrium or intrauterine lesions with a continuous flow resectoscope. Results: 78 patients were followed up for 1 to 18 months. Their menstrual symptoms were ameliorated, of them 20.5% became amenorrhea, 38.5% had little menstruation, 41.0% recovered normal period. The rate of success was 100%. Twelve out of the 14 cases with dysmenorrhea had their pain relieved. Conclusions: hysteroscopic electrosurgery is an effective procedure in treating abnormal uterine bleeding.

    To analyses of the relations between the changes in ultra microstructure of hippocampus amygdala and the temporal lobe epilepsy, the normal preoperational examinations of EEG, V-EEG and MRI were performed in 35 patients with temporal lobe epilepsy. The abnormal hippocampus and amygdala were resected with the intraoperative deep brain monitoring of EEG and the ultra structure examination were performed. Changes of different degrees were observed in ultra microstructure of neurons in epileptic areas. The neuro fiber was slackened and stuck together. The number of excitatory transmitters in presynaptic terminals of axospinal asymmetric synapses was increased. Rough surfaced endoplasmic reticulum(RER)was expland. Astrocytes were swollen mildly. There were more stricking changes in microstructure of all 35 cases which were observed and the changes were unifformed. The pathologic changes of the neuro in the foci play a more important role in the mechanism of the formation and the development of temporal lobe epilepsy.
    Tracheotomy in Treatment of Acute Severe Brain Injuries

    To summarize the application of tracheotomy in treatment of acute severe brain injuries, 37 cases of acute severe brain injury patients with tracheotomy admitted to the department in 1997 were analyzed. All the scores of GCS were 3 to 8 points. Fourteen cases accepted conservative treatment, other 23 cases with emergency operation. Sputum bacterial cultures and drug sensitive tests were routinely carried out after tracheotomy. Twenty seven cases survived and 10 were dead(27.0%). After tracheotomy, the respirations were improved in 35 cases(94.6%)and SaO2 turned normal in 29 cases(78.4%). The bactera of pneumonia infections were commonly drug resistant. Tienam had the lowest resistant ratio(14.3%). Sputum bacterial culture should be done routinely after trachotomy and the drugs can be selected according the result. Transtrocheal medication of antibiotics is a safe and effective methold to treat pulmonary infection.
    Use of Frozen Cranial Vault Allografts in the Repair of Cranial Bone Defects

    To evaluate the effects of cranioplasty using allograft bone flaps, 18 patients with cranial defects were followed up after the original bone flaps were removed. The allograft bone flaps that were stored in a deep frozen state and kept bacterium free were used as repairing materials. All of 18 operations were successful. No infection occurred. X-ray and CT scans showed no absorption of the grafts. The results indicate that cranioplasty using allograft bone flaps is safe and practical with a good cosmetic effects and no infection.

    To compare the neuromuscular block effects of vecuronium administered by continuous infusion or bolus injection, thirty patients undergoing selective neurosurgery were randomly allocated into three groups, 10 in each group. The patients were induced with fentanyl, propofol and vecuronium, 0.1 mg/kg and intubated intratracheally. Anesthesia was maintained with propofol and isoflurane. The neuromuscular function(train-of-four, TOF)was monitored with Biometer acceleromyography. When T1 returned to 2%,the maintaining dose of vecuronium, 0.1 mg/kg, was administrated in three ways: in group A, 0.05 mg/kg iv bolus and repeated while T1=2% again; 0.1 mg/kg iv bolus in group B, and infused intravenously with 0.1 mg/(kg·h)and changed to 0.05 mg/(kg·h)when T1=0. The complete block time(the time between T1=0 and T1=2%)was calculated. The complete block times in group A, B and C were(53.0±3.8)min,(50.3±5.2)min and(102.7±5.5)min respectively, with significant differences among the three groups(P<0.01). Continuos infusion of vecuronium can double neuromuscular block time than bolus injection.

    To compare the changes of coagulation and electrolyte during hypervolemic hemodilution induced by lactated Ringer′s solution(LR), haemaccel(H)or gelofusine(G), 30 neurosurgical patients without heart diseases were randomly allocated into three groups. Hypervolemic hemodilution was induced by rapid infusion, 25 mL/min, in patients of LR(group LR, n=10), H(group H, n=10)or G(group G, n=10)after general anesthesia. Hematocrit(Hct), activate partial thromboplastin time(APTT), prothrombin time(PT)and calcium concentration in serum(Ca2+)were tested after anesthesia induction, infusion of 500 mL, infusion of 1 000 mL, infusion of 1 500 mL, and 30 min after hypervolemic hemodilution, respectively. Results: During hypervolemic hemodilution, Hct decreased by 10.8% after 1 500 mL infusion of LR, while APTT, PT and Ca2+ did not change significantly in group LR. In group H, Hct decreased by 24.1%, Ca2+ increased by 8.3%, APTT and PT did not change significantly after 1 500 mL infusion of H. In group G, Hct and Ca2+ decreased by 25.5% and 12.5% respectively, APTT and PT prolonged 21.0% and 14.0% respectively after 1 500 mL infusion of G. The result shows that H and G used for hypervolemic hemodilution are better than LR; H is suitable for the patients with low serum Ca2+; and G should not be used for patients with coagulation disorder.

    Effects of Hemodilution on Cerebral Blood Flow and Infarction Size in Rat Model of Focal Cerebral Ischemia

    In order to assess the effects of moderate acute normovolemic hemodilution(ANHD)on cerebral blood flow and infarction size in a rat model of focal cerebral ischemia, sixteen Wistar rats were randomly assigned into two groups: control group(n=8)and ANHD group(n=8). The Hct of the animals in ANHD group was reduced to 30.7%. Cortical local cerebral blood flow(LCBF)was monitored in both groups before and during the 120 min occlusion of the left middle cerebral artery with laser Doppler flowmetry. The infarction size of each brain was determined at 24 h after ischemia. LCBF significantly decreased in both groups(P<0.01). The decreasing degree of the ANHD group was significantly lower than that of the contral group(P<0.01), and its cerebral infarction size was significantly smaller than that of the control group(P<0.05 or P<0.01). The results indicate that moderate ANHD increases cortical LCBF and decreases cerebral infarction size in a rat model of focal ischemia. It has a protective effect on cerebral ischemia.

    In order to investigate the method of monitoring the blood concentration of propofol and the propofol parmacokinetics, 22 neurosurgery patients were infused with propofol 2 mg/kg by induction.The infused time was 30 s.After 1,2,3,4,5,10,15,20 min of induction,the blood concentrations of propofol were monitored with high performance liquid chromatography.The time dose relationship shape was charted.The high performance liquid chromatography was a good method to monitor the blood concentration of propofol.It took very short time for propofol to get to peak concentration.After 1 min if induction,the blood concentration of propofol was 9.06 mg/L,and decreased rapidly.This result correlated with the fast induction and quick wake of patients.In this study the blood concentration of propofol was higher than in other study after 1 min of induction,perhaps this is related to infused speed and time.

    To observe the influences of different approaches on stress reaction upon general anesthesia, thirty-one patients ASA grade Ⅰ-Ⅱ, scheduled for selective neurosurgery, were allocated into two groups according to transsphenoidal approach and transcranial approach. Group S(n=16)underwent transsphenoidal approach and group F(n=15)did transcranial approach. Observing points were made at inducting(T1), intubating(T2), placing Hardy-Cushing′s Scope(T3), and destroying the bone of bottom sella(T4)in group S; and at inducting(T1), intubating(T2), peeling off periosteum(T3), sawing skull(T4)in group F. Plasma glucose, serum insulin and glucagon were measured. The value of mean arterial pressure and heart rate were recorded. ln group S,MAP and GLA decreased significantly at the period of T2; and HR, GLU, GLA changed significantly at the period of T3(P<0.01); MAP and INS, and GLA significantly decreased and GLU increased significantly at the period of T4. In group F, MAP and GLA decreased significantly at the period of T2; GLU increased and GLA decreased at the period of T3; HR and GLU increased significantly, and INS decreased significantly at the period of T4(P< 0.01). There are no significant changes in hemodynamics and glycometabolism undergoing transsphenoidal approach and transcranial approach.
